Umbilical hernia is a swelling that can be seen at the umbilicus, or the belly button. It occurs when there is a defect and failure to close a part of the abdominal wall at the umbilical region, thus allowing fluid and parts of the intestine to push through the defect. Hernias should be repaired and closed by surgery.

Background

Muhammad Ibrahim Yarima is a 15-year-old boy living with his parents and relatives in a dilapidated house in Malari ward, Damaturu. He enjoys playing football and dreams of becoming a teacher. He was healthy until about three months ago when he developed swelling around his umbilicus and abdominal pain. Upon examination and investigation at our facility, he was diagnosed with a para-umbilical hernia. He requires appropriate medical treatment.

Medical history

Muhammad had Herniotomy done under general anaesthesia, following a diagnosis of obstructed umbilical hernia. Intra-operatively, he had IV Ciprofloxacin and Metronidazole start while, post-operatively he received IV Tandak 1.5g 12 hourly for 48hours, IV Flagyl 500mg 8hourly for 48hours, IV Pentazocin 30mg 8hourly for 24hours, IM Diclofenac 75mg 12hourly for 48hours. He commenced ambulation and oral sipping after 24 hours. He was converted to oral antibiotics and analgesics and later discharged home after noticing good healing at the surgical site.

Prognosis

Without intervention, the hernia may continue to grow in size, causing increased discomfort, pain, and a noticeable bulge at the umbilicus. In severe cases, the protruding tissue can become trapped or strangulated, which is a medical emergency. Strangulation occurs when the blood supply to the herniated tissue is compromised, leading to severe pain, nausea, vomiting, and possible tissue damage or necrosis. Prompt surgical repair is typically recommended for umbilical hernias to prevent these complications and ensure the safety and well-being of the affected individual.
