Suspected typhoid perforation.Typhoid perforation usually occur as a result of delay in the treatment of typhoid fever, it comes in a forms of complication,and it usually affect the terminal ileum.if it occur there is going to be spillage of contents into the peritoneum and the digestive enzymes may cause more damage to the tissues

Background
Hadiza is an 11-year-old child that has a good character, she like drawing and playing with toys she wanted to become a lawyer. She was well until about 2 weeks before presentation to the hospital when she started complaining of fever and abdominal pain associated with constipation, she was given some drugs which include caps amoxicillin and metronidazole that was bought in the chemist but there was no improvement, then later she was brought to the hospital, At presentation she was febrile with frontal Temp of 39.4,weak and dehydrated. Abdominal examination done showed generalized tenderness with guarding. The patient was looking toxic and was diagnosed with suspected Typhoid perforated.
Medical history
Hadiza was admitted and managed as a case of acute abdomen secondary to Typhoid ileal Perforation. She underwent surgery where the Perforation was repaired and the abdominal cavity was well irrigated. She had a proper post operative management with full course of antibiotics and wound care. She responded well to the treatment before being discharged to resume her normal life. Father was very happy for the support he received from helpster team.
Prognosis
This patient need admission in the paediatrical surgical ward,she need to investigated thoroughly,she need an urgent abdominal USS,E/U/Cr, pass NG Tube for gastric decompression and also put on intra nasal O2 and commence antibiotics and resuscitate,for possible EX Lap The prognosis is good if the patient is treated urgent,but if it is delayed it will lead to multiple perforation,severe septicemia and even death.if it is treated this patient may survive and continue with her school
