Background
Mary, a bright and ambitious young woman, is a partial orphan living with her grandmother and six of her grandmother’s other grandchildren. Although she successfully completed secondary school, financial challenges have prevented her from enrolling in a tertiary institution. During her time in school, Mary demonstrated remarkable talent and leadership, serving as both a class prefect and captain of the football team. She dreams of one day becoming an electrical engineer. At 18, while still in school, Mary had her first child. When she recently began experiencing fatigue and mood swings, she quickly recognized the signs of another pregnancy. Without needing a test to confirm, she informed the baby’s father, who rejected the responsibility and pressured her to terminate the pregnancy - something she refused to do. Around the same time, family tensions escalated between her father and stepmother, resulting in Mary being kicked out of her home. With nowhere else to turn, she went to live with her supportive grandmother. This second pregnancy came shortly after she had completed secondary school, just two years after giving birth to her first child with a different father. Despite the circumstances, Mary attended three antenatal care (ANC) visits at a local dispensary located more than 40 minutes away by foot. At seven months pregnant, during the start of her third trimester, she suffered a severe malaria episode and returned to the dispensary. Lacking adequate resources, the facility referred her to Siaya County Referral Hospital for an ultrasound. Upon arrival, she was admitted as doctors conducted further tests. The ultrasound revealed she had polyhydramnios - a condition involving excessive amniotic fluid - and she was told her baby might not survive the day. An emergency cesarean section was carried out immediately. Mary shared that both her childbirth experiences were through cesarean sections - the first due to prolonged labor. Sadly, neither of her children's fathers visited her in the hospital during or after childbirth, and none have been supportive financially.
Medical history
The child was admitted t the maternity ward as she had started experiencing contractions. She had a prolonged labour and had to be taken for an emergency Caeserean birth, which was a success. She was returned to the ward and was discharged on a orals when stable. A visit to the home by the volunteer revealed that she had recovered and had gone back to her normal life.The volunteer also confirmed that her caeserean wound was healing rapidly and well.
Prognosis
The pelvis was too small and the patient had a history of Caeserian birth due to prolonged labor and thus after examination she was fit to undergo an ERCS which is an elective repeat c-section and was set to have a full recovery as patient was still young and fit
