A second-degree tear in labour is a tear that extends through the skin and muscles of the perineum (the area between the vagina and anus). These tears typically require stitches for repair. Second-degree tears are more severe than first-degree tears (which involve only the skin). Severe anemia, characterized by a significantly low hemoglobin level, can lead to a range of symptoms including extreme fatigue, weakness, shortness of breath, dizziness, and pale skin. In addition, severe anemia can cause more pronounced symptoms such as rapid breathing, heart palpitations, and difficulty concentrating

Background
Stacy is a 16-year-old girl from a large family of 12 children. She is the seventh-born, though sadly, four of her siblings have passed away. She now lives with her parents and three siblings, as her older siblings have since moved out. Her mother describes her as diligent and dependable, often assisting with house chores and work on their small farm. The family resides in a two-roomed house, and both parents earn a living through casual labor, mainly by helping neighbors with farm work like tilling and weeding. Stacy is currently in Grade 7 and aspires to become a teacher. Stacy began feeling unwell over time—constantly tired and sleepy. She confided in her brother’s wife, who bought a pregnancy test kit, which confirmed Stacy was pregnant. This revelation overwhelmed her with stress and anxiety, but her sister-in-law offered her encouragement, and she bravely continued attending school. Her mother, noticing Stacy’s persistent nausea and fatigue, suspected pregnancy and took her to the local Community Health Volunteer (CHV), who accompanied her for antenatal care (ANC). The ANC clinic was about an hour's walk from their home. As her pregnancy advanced, the long walks became more difficult—Stacy often felt dizzy and had to rest by the roadside. Eventually, she was advised to avoid walking long distances and only attend the clinic if she could afford motorbike transport. One day, she began experiencing what she thought were minor cramps at 8:00 a.m., but the pain continued throughout the day. Unaware they were contractions, she endured the discomfort until late at night when it became unbearable. Her mother contacted the CHV, who urged them to get to the nearest dispensary. Stacy delivered her baby at the dispensary, but sustained a deep vaginal tear and was extremely weak due to heavy blood loss. She was urgently referred to Siaya County Referral Hospital, where she was admitted through the emergency department, received a blood transfusion, and was stabilized by medical staff.
Medical history
Stacy was brought to the hospital a referal from a local dispensary where she had suffered a second degree tear during childbirth and,in the process, lost a lot of blood. She was admitted for a day where her tear was sutured and was transfused blood to help her recover from her blood loss. She was discharged on Caps Amoxil 500g tds for 7 days 4. Tabs ferrous sulphate 200mg tis for 4 weeks Tabs Flagyl 400mg tds for 7 days 5. Tabs panadol ll tds for 3 days. A visit to her home by the volunteer confirmed that Stacy was healed and was able to help her grandmother with day to day house chores
Prognosis
Prognosis is good since she was transfused and the tear repaired to prevent further blood loss
