Background

Lokwayen Akalale is a 14-year-old boy from the remote plains of Turkana County, raised in a pastoralist family where life revolves around herding and searching for water. He grew up active and curious, helping with daily chores and enjoying singing traditional songs. He dreams of becoming a veterinary doctor to care for animals in his community. At the age of 8, he began experiencing fatigue, chest pain, and difficulty breathing, which worsened over time. After a long journey to a health facility, he was diagnosed with rheumatic heart disease, a chronic condition caused by untreated infections. Since then, his health has declined, limiting his ability to participate in normal activities, while his family struggles to afford ongoing care. In recent weeks, his condition deteriorated, with cough, fever, night sweats, loss of appetite, abdominal swelling, facial puffiness, and ankle edema. He became very weak and developed severe breathing difficulty and chest pain. Although Lokwayen has a chronic illness, he presented to the hospital in a critical emergency state, requiring immediate intervention to stabilize him and save his life before referral for specialized care.

Medical history

At age 8, Lokwayen began showing alarming symptoms such as abdominal swelling, a puffy face, and swollen ankles. His condition gradually worsened—he became easily fatigued, struggled to breathe, and eventually collapsed during a village event. What was first thought to be dehydration or exhaustion was upper respiratory tract infection and later confirmed to be rheumatic heart disease (RHD), a preventable condition caused by untreated throat infections that had severely affected his heart valves. He was started on medication to help reduce the excess fluid that had built up in his body. Lokwayen is now being referred to a higher-level facility for a cardiologist’s evaluation and an echocardiography. Lokwayen’s outlook depends greatly on how quickly and consistently he is able to receive specialized medical care, especially given the significant valve damage already caused by RHD.

Prognosis

Lokwayen presented with acute decompensated heart failure on a background of rheumatic heart disease, likely precipitated by a respiratory tract infection. He was in a critical state with breathing difficulty, weakness, and fluid overload, requiring urgent stabilization with oxygen, diuretics, and supportive care. He requires ongoing management under a cardiologist, including evaluation for long-term medical therapy and possible surgical intervention.
