Severe malaria, a life-threatening complication of malaria infection, is characterized by organ dysfunction, including impaired consciousness, seizures, circulatory collapse, and respiratory distress. Other key features include severe anemia, metabolic acidosis, acute kidney injury, and abnormal bleeding or disseminated intravascular coagulation (DIC). Early and aggressive treatment is crucial for survival. Acute kidney injury (AKI), formerly known as acute renal failure, is a sudden decline in kidney function that develops within a few days. It's characterized by a rise in blood creatinine levels and/or a decrease in urine output, or both. AKI can be caused by a variety of factors, including prerenal (low blood flow to the kidneys), intrinsic renal (damage to the kidneys themselves), or postrenal (blockage of urine flow) issue.

Background
Liandro is a 3-year-old girl living with her grandparents in a one-room mud house in Pap Nyadiel, Kenya. She was abandoned by her biological mother when she was just two months old. Her grandparents, who are elderly, rely on small-scale subsistence farming for their livelihood. According to her grandmother, Liandro is normally cheerful and playful when she's well, especially around familiar people, but becomes quiet and reserved around strangers. Recently, Liandro developed a persistent cough, for which her grandmother administered over-the-counter medication, but there was no improvement. The following day, her condition worsened—she began vomiting, lost her appetite, appeared generally weak, and experienced multiple convulsions throughout the day. By evening, her condition had deteriorated so much that she became unresponsive. Alarmed, the grandmother sought help from a neighbor who, upon seeing Liandro’s condition, gave her money to hire a motorbike to rush the child to the hospital. They arrived at the emergency department at 8:00 p.m.
Medical history
Liandro was brought to the hospital exhibiting coughs, vomiting, general body weakness and convulsions. Her tests were run in the lab, and she was diagnosed with acute kidney injury and severe malaria. These conditions warranted admission, and she was promptly admitted to the ward for proper and close management.She was treated and she responded well to treatment and was eventually discharged on orals like paracetamol, AL an antimalarial), and ORS drug for every time she had an episode of vomiting. A visit by the volunteer to her home revealed that Liandro was okay, the volunteer found Liandro playing with her neighbours an indication she had gone back to her normal life.
Prognosis
prognosis of acute kidney injury (AKI) vary depending on the severity, underlying cause, and individual patient factors, but generally, a significant number of AKI cases recover completely if treated promptly. However, there's also a risk of developing long-term complications like chronic kidney disease and increased mortality. The patient was brought to the facility on time before her kidneys deteroriated, with prompt management administered the child is set to make a full recovery
