Background

Stacy is an 18-year-old orphan who lives with her grandmother and cousin in a small two-room iron sheet house. She was recently attending school, where she was active in the music club and played handball. Stacy relies on her grandmother for support, a peasant farmer with no stable source of income, and the family survives on a day-to-day basis. Stacy dreams of becoming a teacher once she completes her education. After feeling unwell and missing her periods, Stacy suspected she might be pregnant. She shared her concerns with a friend, who bought her a pregnancy test kit—confirming her suspicion. This happened during the school holidays. Encouraged by her friend, she returned to school. When she informed the father of the child, he didn’t respond but just walked away. Stacy continued with school while keeping the pregnancy a secret from her grandmother. At six months, when it became visible, she disclosed it to her grandmother, who was upset but advised her to start attending antenatal care (ANC) visits. Stacy managed to attend three ANC appointments, walking over 40 minutes each way. One early morning at 4:00 a.m., she began experiencing contractions and went to the nearest Dispensary. Due to the complications, she was referred to Siaya County Referral Hospital, where she was admitted to the labor ward. After a prolonged labor, she was taken to the operating room for caesarean section.

Medical history

Stacy was brought to the hospital with severe back pains. She had started experiencing her contractions, and she was immediately admitted to the maternity room. Her labor continued but failed to progress, thus experiencing a prolonged labor. This caused a risk to both the mother and baby, and she had to be rushed to the theater for an emergency C-section. The surgery was successful, and she was returned to the general ward, where she was closely monitored for any postoperative complications, of which there were none. Stacy was later discharged on orals. A call to her revealed that she had recovered, her wound had healed fine, and she could move around with ease and even help with some house chores.

Prognosis

Prolonged labor can have negative effects on both mother and baby. In the first stage of labor, it can lead to exhaustion and mental distress, while in the second stage, it increases the risk of maternal complications like infection, postpartum hemorrhage, and future incontinence, as well as the potential for uterine rupture. The patient was promptly managed before further complications arose
