A pregnancy at 30+ weeks with a history of one prior lower segment caesarean section (LSCS) is classified as high-risk due to the increased likelihood of uterine scar dehiscence or rupture, particularly during labor. If not managed with timely surgical intervention, such as an elective repeat caesarean section (ERCS), it can lead to catastrophic obstetric complications including fetal hypoxia, intrauterine fetal demise, maternal hemorrhage, or even maternal mortality.

Background
Sumaiya Akter, a 27-year-old housewife, is 30+ weeks pregnant and has been advised by doctors to undergo a cesarean section due to her previous surgical delivery. She lives with her husband, young son, and elderly in-laws, relying solely on her husband's income of 8000–9000 Taka from day labour at a local biscuit factory. The family struggles to meet basic needs and cannot afford private medical care, while government hospital facilities are too far and inadequate for her condition. Sumaiya is known as a calm and devoted mother who takes care of her family despite their hardships. Without the necessary surgery, she and her unborn baby face life-threatening complications during delivery.
Medical history
After experiencing less fetal movement for over 38 weeks, she arrived at our hospital where doctors performed an ultrasound that revealed fetal distress , making vaginal delivery impossible. An emergency cesarean section was promptly arranged, and the procedure was successful, resulting in the birth of a beautiful male baby. Both the mother and the baby are safe and sound, with no complications arising from the surgery. The patient's family expressed their heartfelt gratitude to the Helpster Heroes and the hospital for their timely and effective care.
Prognosis
The patient, with a history of one previous lower segment caesarean section, is currently 30+ weeks pregnant and has been advised to undergo an elective repeat caesarean section (ERCS) to avoid complications. The diagnosis is an antenatal case with a prior LSCS, which is a clear indication for repeat caesarean due to the risk of uterine rupture during labor. If timely surgical intervention is not performed, there is a significant risk of maternal and fetal morbidity or mortality.
