Chesang is a 20-year-old young woman who separated from her spouse, who was also 20. They got married while still in secondary school, which led both of them to drop out. After the separation, Chesang returned to live with her parents. Her father is a small-scale farmer whose limited earnings support the entire family’s daily needs. They live in a modest three-room house, fetch water from a nearby stream, and share a pit latrine with their neighbors. Chesang is an active and hardworking girl who helps her father with the farm work. She dreams of becoming a fashion designer in the future. A day ago, Chesang began experiencing lower abdominal pain and backache. The next day, her condition worsened, and she was rushed to the hospital. She was found to be in the second stage of labor with a breech presentation. Sadly, she delivered a fresh stillbirth and experienced postpartum hemorrhage, which was managed by the medical team. Breech presentation is a medical emergency during delivery because the baby’s feet or buttocks come out first instead of the head, increasing the risk of complications. This abnormal positioning can lead to umbilical cord prolapse, where the cord slips out ahead of the baby, cutting off oxygen. It also poses a high risk of birth trauma, delayed delivery of the head, and asphyxia, especially if not managed by skilled personnel or if vaginal delivery is attempted without proper assessment. Unfortunately, Olivia presented to the hospital a little too late and lost the baby. Postpartum haemorrhage (PPH) is an emergency because it involves excessive bleeding after childbirth, typically more than 500 ml after vaginal delivery or 1,000 ml after cesarean section. If not promptly controlled, PPH can lead to hypovolemic shock, organ failure, or death.

Olivia chesang came to the facility with 2nd stage of labour with complains of abdominal pain, backche and headache on examination , the baby presentation was buttocks rather than in the typical head-first orientation, when was already full dilated and have fresh still birth the mother was stable when discharged.

Prognosis is good; the excessive bleeding was managed well Olivia was advised to attend antenatal care (ANC) regularly so that conditions like breech presentation can be detected early. She was also encouraged to always go to the hospital promptly when labor begins.
