Background

The family comes from a humble background in a rural setting. They rely on a nearby stream as their source of water and share a toilet facility with other households. The whereabouts of the father are unknown, and the mother sustains the family through casual labor, earning less than KSh 100 a day—insufficient to cover basic needs. Miriam, who was pregnant as per the time she came to the hospital, was admitted with complaints of per vaginal bleeding that began suddenly and progressively worsened, though the exact amount of bleeding could not be measured. The bleeding was accompanied by lower abdominal pain radiating to the back, with the pain increasing gradually in intensity and frequency. It’s worth noting that she has a history of a previous surgical scar from 2024.

Medical history

Miriam, who was pregnant at the time of presentation, was admitted with complaints of sudden-onset vaginal bleeding that progressively worsened, though the exact amount could not be quantified. The bleeding was associated with lower abdominal pain radiating to the back, which gradually increased in both intensity and frequency. She also had a history of a previous surgical scar from 2024. On admission, an obstetric ultrasound revealed an intrauterine fetal death (IUFD). She subsequently underwent an emergency caesarean section, after which she received a blood transfusion and was commenced on intravenous antibiotics and analgesics. She progressed well after the caesarean section, to recovery and was discharged. A follow up phone call confirmed that she was doing well

Prognosis

She underwent an emergency cesarean section, which successfully managed the acute condition and stabilized both the patient and the pregnancy outcome. With timely caesarean section intervention, her immediate prognosis and that of the newborn is favorable.
