Severe oligohydramnios is a serious pregnancy complication characterized by an unusually low amount of amniotic fluid, defined by ultrasound measurements of the largest pocket of amniotic fluid being less than 1 cm. Oligohydramnios in general is when the amniotic fluid volume is less than expected for the gestational age, with severe cases indicating a particularly low amount. A repeat C-section is required for a second delivery due to factors like the type of scar from the previous C-section, particularly a vertical incision, or if there's a history of uterine rupture, as these situations significantly increase the risk of uterine rupture during a vaginal birth.

Background
Yasmin Begum, a 35-year-old housewife, lives in poverty with her husband, Abdur Rahman, a 40-year-old day laborer earning only 6,000 BDT a month, their 8-year-old son, Yasin, and Abdur Rahman’s elderly mother, who is unable to work. She is 35 weeks pregnant and suffers from severe oligohydramnios, a dangerous condition with very low amniotic fluid, while her first delivery was by C-section. Doctors have advised an urgent repeat C-section to save both her and her baby’s lives, but the family cannot afford the cost due to their meager income and daily struggle for food, medicine, and rent. Yasmin is a gentle, caring woman who loves cooking for her family, yet without treatment, the risk of complications such as uterine rupture, birth trauma, or stillbirth is extremely high. Without urgent help, her son could lose his mother, the unborn baby’s life could be lost, and the entire family would face unimaginable grief and hardship.
Medical history
Yasmin Begum was admitted to the hospital on August 08, 2025, with a diagnosis of severe Oligohydramnois. On the same day, she had a successful cesarean section and gave birth to a healthy baby. Both mother and newborn were discharged on August 12, 2025, in good health. Her husband came to the hospital to receive her and their child. Prognosis
Severe oligohydramnios can indicate underlying issues and may lead to serious complications for both the mother and the baby, including impaired fetal lung development, limb contractures, and problems during labor and delivery. This is why it is urgently necessary to perform a C-section, especially as her first delivery was also by C-section. Without it, both mother and child could face life-threatening situations. A repeat Cesarean section is often recommended after one or two prior C-sections due to increased risks in subsequent pregnancies, such as complications with the placenta and uterine rupture, making a planned C-section the safer option in most cases. For these reasons, performing the C-section is essential.
