Oligohydramnios is a pregnancy complication characterized by abnormally low amniotic fluid, which is crucial for fetal lung development, movement, and protection. Without timely intervention (such as C-section or close monitoring), it can lead to severe birth defects, umbilical cord compression, stillbirth, or life-threatening complications for both mother and baby during delivery. Fetal Intrauterine Growth Restriction (IUGR) is a condition where a fetus is smaller than expected for its gestational age due to inadequate growth within the uterus. If not properly managed, IUGR can lead to serious complications such as fetal hypoxia, birth asphyxia, meconium aspiration, or stillbirth, posing significant risks to the baby's health and survival.

Background
Rumana Akter, a 22-year-old pregnant woman from a very poor family, experienced prolonged labor at home that lasted over 20 hours without progress. Her husband Md. Samim, a daily wage laborer earning only 8,000 Taka per month, and her mother-in-law Champa Begum, struggle to afford proper medical care. When ultrasound revealed moderate oligohydramnios and fetal distress, they knew urgent intervention was needed but could not afford the high costs of private treatment. Relying on the free services offered by Helpster at Shaheda Gafour Ibrahim Hospital was their only hope; without timely cesarean delivery, Rumana and her baby could face life-threatening complications such as fetal death or maternal hemorrhage. The family’s limited income and lack of proper healthcare facilities put both their lives at serious risk.
Medical history
After experiencing labor pain for over 38+ weeks, she arrived at our hospital where doctors performed an ultrasound that revealed that amniotic fluid is below low and there is fetal distress of the baby, making vaginal delivery impossible. An emergency cesarean section was promptly arranged, and the procedure was successful, resulting in the birth of a beautiful baby girl. Both the mother and the baby are safe and sound, with no complications arising from the surgery. The patient's family expressed their heartfelt gratitude to the Helpster Heroes and the hospital for their timely and effective care.
Prognosis
Rumana Akter, a primigravida at full term with prolonged labor, moderate oligohydramnios, and fetal distress, requires urgent cesarean section to prevent further deterioration of both her and her baby's health. Prompt surgical intervention is essential to reduce the risk of fetal hypoxia, fetal demise, and maternal complications such as hemorrhage or uterine rupture. Delaying the procedure could result in life-threatening outcomes for both mother and baby.
