Background

Gift is the youngest child in his family. He lives in a modest two-room mud house with his three siblings and parents. The family survives on informal work; the father takes up casual construction jobs when available, while the mother earns a small income doing laundry for others or helping with farm work in neighbors’ fields. According to the mother, she noticed Gift felt hot to the touch while serving him food and seemed unusually quiet and uninterested in eating. After the meal, he experienced several episodes of diarrhea and developed a persistent cough. Hoping he would improve, she gave him paracetamol at home. However, his condition did not get better even after a week. Concerned, she finally decided to take him to the hospital, borrowing transport money from a neighbor to get him there.

Medical history

Gift was brought to the hospital exhibiting symptoms like vomiting,coughing and passing of loose stool. Investigations were done and he was found to be anaemic, suffering from malaria and was malnourished. These results warranted admission and he was admitted for a number of days for management. In the ward he was given IV Artesunate for his malaria, X-pen . He was also given ORD to make up for the minerals and fluids lost and he was also transfused blood for his anaemia. Gift responded well to treatment and was discharged on orals and was advised to visit the hospital at a later date for review. A visit to Gift's home by the volunteer confirmed that he was indeed well and was able to play as before.

Prognosis

This is a life-threatening condition requiring urgent hospital admission and immediate intervention. The clinical presentation suggests significant red blood cell destruction from the malaria infection, contributing to the severe anemia. The combination of these two conditions increases the risk of circulatory collapse, hypoxic injury, and multi-organ dysfunction, particularly in young or malnourished children.
