Why is treatment sometimes done earlier than the payment?
When a case is urgent and vital, hospitals often proceed with delivering medical care right away to save a life, even before the finalisation of admission procedures. Usually, when this happens, hospitals put the bill on our credit until the funds are transferred to their accounts. That’s why the chronology can sometimes be in disorder.

About enteric fever + malaria
Enteric fever and malaria are two common infectious diseases in tropical regions, often presenting with fever but caused by very different pathogens and transmitted through distinct routes. Enteric fever, caused by Salmonella typhi or paratyphi and spread via contaminated food and water, is characterized by sustained high fever, abdominal pain, gastrointestinal disturbances, loss of appetite, and generalized weakness, with risks of intestinal perforation or bleeding if untreated. Malaria, on the other hand, results from Plasmodium parasites transmitted by the bite of infected Anopheles mosquitoes, producing cyclical fever with chills, sweating, headaches, anemia, and potential complications like cerebral malaria or multi-organ involvement in severe cases.

Background
Umar is 7 years old, he is the 6th child among 8 children, his parents couldn't send him to school because they cannot afford his school fees, the parents are not having any job at the moment. They live in a 2 rooms apartment, they drink well water, no electricity and no good road to their home. They are struggling to feed. They couldn't afford to take Umar to the hospital and he has been sick for about 8 days. Umar has been inactive for days, having high body temperature, history of high-grade fever, headache and abdominal pain with associated loss of appetite, only some paracetamol was given to Umar but no positive changes in his health, the parents doesn't have the money for the medical bill. They were told about Helpster Charity by a group of some community women and they reached out to Helpster Charity partner named WholeCare Hospital where he was evaluated respectively. Upon presentation at our hospital and after thorough investigation he was diagnosed of Malaria and Enteric fever
Medical history
Umar-a 7 years old child was admitted with high grade fever, inactive and not interested in any food. He was thoroughly examined and investigations revealed malaria and enteric fever. Some antimalarials and antibiotics as well as other supportive care were given to him. When he became stable, he started eating well, became active and regained his strength, he was discharged with some take home drugs and was given a date for follow up. They are happy and thankful to Helpster Charity.
Prognosis
Because both illnesses share fever as a common symptom, proper diagnosis through laboratory tests—such as blood films for malaria parasites, rapid diagnostic kits, and cultures or serology for typhoid—is vital to distinguish them. Prompt and targeted treatment with appropriate antimalarials or antibiotics, along with supportive care, ensures effective recovery and prevents life-threatening complications. Umara had worsening enteric fever that could have led to intestinal perforation if not promptly treated. He also had malaria that might have been triggered by the enteric fever.
