This patient is in a high-risk pregnancy situation at 38+ weeks. The presence of oligohydramnios, which is a dangerously low level of amniotic fluid, is a significant concern as it can put the baby at risk for complications like umbilical cord compression. Compounding this, the patient felt less fetal movement without labour pain which indicates fetal distress has started, which makes her current delivery more complex and necessitates C-section. This combination of factors requires close medical supervision to ensure the safety of both the mother and the baby.

Background
Rima Akter, a 25-year-old housewife and loving mother of five-year-old Fabiha, lives with her husband Faisal, a daily wage laborer earning only 9,000 BDT per month, and her elderly mother Ankez Begum. Now pregnant for the second time, Rima suddenly experienced a gush of water and reduced fetal movement at 38+ weeks, prompting an urgent visit to the hospital. The doctors found severe oligohydramnios with fetal distress and recommended an immediate cesarean section to save her baby’s life. With such a low family income, covering the cost of surgery is nearly impossible, putting both mother and child at grave risk. Without timely intervention, the unborn baby could suffer permanent harm or even death, making urgent surgical care critical for Rima and her family.
Medical history
After experiencing, passage of gush of water and leaky membrane for 3days and less fetal movement for 1 day without labour pain, she arrived at our hospital at 1.00PM in 29 September 2025 and doctors performed an ultrasound that revealed amniotic fluid is very low with fetal distress , making vaginal delivery impossible. An emergency cesarean section was promptly arranged, and the procedure was successful, resulting in the birth of a beautiful male baby. Both the mother and the baby are safe and sound, with no complications arising from the surgery. We discharged the patient at 10.30AM in 02 October 2025.The patient's family expressed their heartfelt gratitude to the Helpster Heroes and the hospital for their timely and effective care.
Prognosis
The doctor's prognosis is that an immediate cesarean section is a crucial, life-saving intervention. The patient's history of premature rupture of membranes, severe oligohydramnios, and reduced fetal movement puts both the mother and baby at significant risk. The lack of amniotic fluid can lead to the umbilical cord becoming compressed, which can cut off the baby's oxygen supply and lead to fetal distress, irreversible brain damage, or stillbirth. Without the immediate surgical delivery, the risk of a tragic outcome for the baby is high. The C-section, while a major surgery, offers the best chance to bypass these life-threatening complications and ensure a safe and healthy start to the baby's life.
