Gastroenteritis Malaria with Enteric fever is a combined infection where Plasmodium parasites transmitted by mosquito bites cause high fever, chills, headache, weakness, and anemia, while Salmonella typhi leads to prolonged fever, abdominal pain, and loss of appetite. Otitis Media is an infection of the middle ear that presents with ear pain, fever, irritability, and sometimes ear discharge. Gastroenteritis is an infection or irritation of the stomach and intestines, causing diarrhea, vomiting, abdominal cramps, dehydration, and general weakness.

Background
Mohammed Bashir is a 1 year old boy who lives with his parents and four siblings in a crowded one-bedroom apartment. The family faces very poor living conditions, with limited resources to meet their daily needs. Little Mohammed Bashir, usually bright and playful, began to suffer from serious abdominal pain, frequent loose stools, fever, body aches, and ear pain. His condition worsened quickly, leaving him weak and unable to play as usual. His poor parents, who could not afford proper treatment at home, became very worried and decided to take him to Nakowa for urgent medical support. On presentation at our hospital, and after further evaluation, he was diagnosed with Malaria + Enteric fever,Otitis media and acute Gastroenteritis.
Medical history
At Nakowa, Mohammed Bashir was treated for Malaria with Enteric fever, Otitis Media, and Gastroenteritis. His treatment included Syrup Amoxicillin 5ml tds for one week, Tablets Artemeter 1 bd for three days, Tablets Zinc 10mg daily for ten days, Syrup Vitamin B Complex once daily for one week, Syrup Paracetamol 5ml for three days, Ciprofloxacin ear drops bd for five days, and Syrup Metronidazole 5ml bd for one week. In addition, he received intramuscular injections of Emal 40mg daily for three days and Paracetamol 150mg, supported with syringes for administration. After starting treatment, his fever reduced, his abdominal pain eased, and his loose stools gradually stopped. The ear drops relieved his ear pain, and his strength returned day by day. On follow-up, he was once again cheerful and active, back to his normal playful self.
Prognosis
With prompt diagnosis and appropriate treatment, Mohammed’s conditions — malaria with enteric fever, otitis media, and gastroenteritis — generally have a good prognosis. Full recovery is typically achieved with the administration of antimalarial agents, appropriate antibiotics, rehydration therapy, and supportive care. However, if treatment is delayed or inadequate, complications may arise, including severe anemia, septicemia, dehydration, electrolyte imbalance, and hearing loss secondary to untreated otitis media. In rare cases, these illnesses can progress to life-threatening systemic disease. Early and timely medical intervention substantially reduces these risks and greatly improves Mohammed’s chances of complete recovery.
