Severe malaria is a life-threatening medical emergency that occurs when a malaria infection, most commonly caused by the parasite Plasmodium falciparum, progresses to a complicated stage. It is defined by the presence of one or more signs of vital organ dysfunction or other severe complications, in addition to the presence of malaria parasites in the blood. If left untreated, it can rapidly lead to death. Symptoms and Complications The symptoms of severe malaria are a result of the parasite's effect on various organs and bodily systems. They can include: * Cerebral malaria: This is a neurological syndrome characterized by impaired consciousness, lethargy, confusion, or coma. It's often accompanied by convulsions or seizures. * Severe anemia: A significant drop in red blood cell count, which can lead to extreme tiredness, weakness, and breathing difficulties. * Acute respiratory distress syndrome (ARDS): Fluid accumulation in the lungs, making breathing difficult. * Acute kidney injury: Renal impairment that can be indicated by a decrease in urine output and high levels of waste products in the blood. * Hypoglycemia: Abnormally low blood sugar, which is particularly common in children and pregnant women. * Acidosis: An imbalance in the body's acid-base level, which can manifest as rapid, deep, and labored breathing. * Jaundice: Yellowing of the skin and eyes, caused by liver dysfunction. * Circulatory collapse/shock: A sudden drop in blood pressure, leading to inadequate blood flow to the body's tissues. Treatment Immediate treatment is crucial for severe malaria. The primary goal is to kill the parasites and manage the life-threatening complications. * Parenteral Antimalarials: The recommended first-line treatment for severe malaria is intravenous (IV) artesunate. This drug is highly effective and acts quickly to reduce the parasite load. In some cases, other drugs like artemether or quinine may be used as alternatives, but IV artesunate is generally preferred. * Supportive care: In addition to antimalarial medication, patients with severe malaria require comprehensive supportive care. This may involve: * Managing breathing difficulties. * Treating seizures. * Correcting low blood sugar. * Administering blood transfusions for severe anemia. * Managing kidney failure. * Follow-up: Once the patient can tolerate oral medication and has received at least 24 hours of parenteral therapy, treatment is typically completed with a course of an artemisinin-based combination therapy (ACT). Risk Groups While anyone can get severe malaria, certain groups are at a higher risk of developing severe complications and a fatal outcome. These include: * Children under five years of age. * Pregnant women. * Travelers from non-endemic areas. * Individuals with a weakened immune system. It's important to seek emergency medical attention immediately if severe malaria is suspected, as a delay in treatment can be fatal.

Background

Abubakar of 7 years is an orphan boy who leave with his grandmother he only attended Islamic school from amshi village. He started having fever for over a week , followed by abdominal pain and vomiting. Upon presentation at our hospital ,he was diagnosed with Malaria The initial symptoms of malaria in children can be non-specific, resembling other common childhood illnesses like the flu, gastroenteritis, or even pneumonia. The girl may have started with a high fever, headaches, chills, nausea, vomiting, and extreme weakness. However, without treatment, the disease progresses quickly from uncomplicated to severe, often within a matter of hours.

Medical history

Abubakar’s was diagnosed with severe malaria. He was admitted and immediate treatment started with appropriate medications. The patient had received an immediate intravenous (IV) dose of Artesunate (2.4 mg/kg). This remains the first-line treatment for severe malaria because it clears parasites from the bloodstream significantly faster than other drugs. He returned to playing with his friends, his laughter once again echoing through the village. His parents, forever grateful to the Helpster charity and the medical staff for the life-saving treatment.

Prognosis

severe malaria is a medical emergency with a grave prognosis if not treated promptly. The opinion of doctors and healthcare professionals is that immediate and aggressive treatment, particularly with injectable artesunate and comprehensive supportive care, is absolutely essential to improve the patient's chances of survival and minimize long-term complications.
