When a case is urgent and vital, hospitals often proceed with delivering medical care right away to save a life, even before the finalisation of admission procedures. Usually, when this happens, hospitals put the bill on our credit until the funds are transferred to their accounts. That’s why the chronology can sometimes be in disorder.

Hypospadias - A congenital (present at birth) condition in males where the opening of the urethra (the meatus) is located on the underside of the penis instead of at the tip. It can vary in severity, with the opening found anywhere from just below the tip to the scrotal area. Undescended testes (cryptorchidism) – A condition in which one or both testicles fail to move down into the scrotum before birth. The testis may be located in the abdomen, groin, or along the normal path of descent, but not in its proper position in the scrotal sac. Findings indicate a short penile length (4 cm) with chordee (downward curvature), subcoronal hypospadias (urethral opening just below the glans), an adequate urethral plate, hypoplastic (underdeveloped) scrotum, and bilateral undescended testes located in the inguinal canals.

Leon Mido is a lively and bright 3-year-old residing at a Children’s Home, where he is cared for through the support of well-wishers. Having just started school this year, he enjoys colouring, singing, and dancing. Curious and full of ambition, Leon dreams of becoming a doctor so he can help others, and with continued support, he has great potential to achieve this goal. Recently, Barika Medical Centre, together with a Helpster Charity volunteer, Vincent, visited the children’s home to introduce Helpster Charity’s work. During the visit, the team explained the organization’s mission, the kind of support available, and how their programs benefit children in need, with the aim of creating awareness and strengthening collaboration with the home. Leon, who was taken into the Children’s Home just a few weeks after birth, was found by the home’s nurse to have a medical condition affecting his genital area that required specialist intervention. Despite efforts over the past three years, the home has been unable to raise the funds needed for the corrective surgery. His case was recently referred to Barika Medical Centre, where, through the support of Helpster Charity, the home was assured that Leon would finally receive the surgical intervention he urgently requires. This support offers renewed hope for his full recovery, enabling him to grow and thrive in good health, free from the challenges of his current condition.

Medical history

Leon Mido was admitted with subcoronal hypospadias, a condition where the opening of the urethra is located on the underside of the penis instead of at the tip. He underwent a successful tubularized incised plate (TIP) urethroplasty, a surgical procedure performed under general anesthesia to reconstruct the urethra and reposition the urinary opening to its normal location. A urinary catheter was inserted after surgery to support healing. Following the operation, Leon was closely monitored in the hospital. Laboratory investigations were normal, and no complications were reported. The surgical site was cleaned and dressed, and he received medications for pain relief, infection prevention, and wound care. Leon recovered well after surgery and was discharged in stable condition. He was advised to continue his prescribed medications, maintain proper wound care, and attend a follow-up review appointment. The outcome of the treatment is expected to improve his urinary function, appearance of the penis, and overall quality of life.

Prognosis

The child has subcoronal hypospadias with chordee, hypoplastic scrotum, and bilateral undescended testes. With early staged surgery (to straighten the penis, reposition the urethra, and bring the testes into the scrotum), prognosis is generally good for normal urination, appearance, and future sexual function. Untreated, the condition can lead to abnormal urination, curvature, infertility, and higher testicular cancer risk. Recovery involves pre-surgical assessment, one or more operations, short-term catheter use, and long-term follow-up into puberty.
