When a case is urgent and vital, hospitals often proceed with delivering medical care right away to save a life, even before the finalisation of admission procedures. Usually, when this happens, hospitals put the bill on our credit until the funds are transferred to their accounts. That’s why the chronology can sometimes be in disorder.

About right lumbar intradural dermoid
This is a rare, benign congenital tumor arising from trapped embryonic ectodermal tissue within the spinal canal. It is located intradurally in the lumbar region and may contain hair, sebaceous material, or fat. Patients often present with back pain, neurological deficits (weakness, numbness), or bladder/bowel dysfunction due to spinal cord or nerve root compression. Hannatu was presented with playful signs and no form of stress, however the mass continues to increase as she grows. In her current stage, it will be good for timely excision biopsy.

Background
Hannatu is the last child among 4 children. She was born 4 months ago. Her family are so happy she came into their lives. The father is a small scale farmer in their small community while the mother is a house wife. The father is struggling so hard to provide for the family, he had to sell his phone to even transport his family to Bauchi, WholeCare hospital which is about 200 km away from his home. They live in a 2 rooms apartment, and a toilet just outside the house. Hannatu was born with a mass that started small on the right flank just to the right of spinal column and has been increasing as the patient grows. No fever or localizing sign. Pregnancy was not eventful, no similar problem in the family. Examination shows a playful baby not in any form of distress. A mass measuring 11x16cm on her right posterior back at the margin of lumbar vertebral column. Patchy hyperpigmented overlying skin noted. The father is so worried there is no money to take her to the hospital, he was informed about Helpster from the mosque. On presented at our hospital, the baby was examined thoroughly and after due investigations was diagnosed with Right Lumbar Intradural Dermoid. She is planned for Excision Surgery upon approval.
Medical history
Hannatu, an 8-month-old female child was admitted on account of right lumbar mass since birth. Mass was soft, nontender located just to the right of the midline. MRI showed a right lipomeningocele. She had excision of the lipoma and repair of the dura. She had CSF leakage with partial wound dehiscence which necessitated secondary closure. Her feeding was noticed to be poor to the extend that she had oral thrush from poor nutrition. She underwent nutritional rehabilitation. Patient eventually did well and was discharged after 25 days on admission. She is encouraged to follow post operative instructions, take medications as prescribed and to keep wound side clean as well as continue nutritional rehab. To keep to scheduled follow-up visit as discussed. The parents are grateful for the treatment given to their daughter.
Prognosis
Complete surgical removal usually offers good long-term outcomes, though adherence to neural tissue can make excision difficult. Recurrence may occur if remnants of the cyst wall are left behind. Neurological recovery depends on the extent and duration of spinal cord or nerve compression before surgery.
