Background

Susan is a 17-year-old girl from a humble rural background. She is the firstborn in a family of six and the family relies on their father, a casual laborer on a neighbor’s farm, earning approximately KSh 4,500 per month, which he uses to meet the children’s school fees and basic needs. Susan is a hardworking and self-motivated student in Grade 9. She became pregnant while still in school due to the challenges of poverty, but she hopes to resume her education in the future. Susan presented to the facility with lower abdominal pain radiating to her back for the past two days, with increasing frequency and intensity. She reported fluid discharge about six hours prior and had been on treatment for a urinary tract infection in pregnancy over the past two weeks. She was at 39 weeks gestation with 5 cm cervical dilation that had failed to progress, accompanied by reduced fetal movement. On examination, meconium grade 2 was noted, and she was diagnosed with fetal distress due to prolonged second-stage labor. An emergency cesarean section was performed to ensure the safety of both mother and baby. The procedure was successful, and both mother and newborn are in stable condition.

Medical history

Susan came to the facility with complains of, lower abdominal pain,lap that radiating to the back, increasing in frequency and intensity, having draining liquid for about 6 hrs and she was on management for UTI for the last two weeks, she went through examination and laboratory investigation and found that she had meconium grade 2 with fetal distress condition. Ceaserian section has to be conducted as the only safest method of saving lives of mother and the baby, the operation was successful both mother and baby were stable. Mother was put on i.v N/S, 5%D, i.v cef, i.m tramadol among other drugs, she responded positively to medication and recovered after awhile and discharged at a stable condition. Susan and her baby came for check up at the hospital 5 days after discharge; they are doing well and Susan is full of gratitude to Helpster Charity

Prognosis

Prolonged labour with fetal distress is an emergency cases that need agent action for the purpose of saving lives. If delays occured or late intervention it could have been resulted to complications such as increase the risks of infection, hemorrhage, and uterine rupture for the parent, while the baby may face oxygen deprivation, resulting in brain injury (like cerebral palsy) and nerve damage .
