Kulsum Begum, a 32-year-old first-time mother, is a caring housewife who dreams of a peaceful and happy life with her husband, Aliullah, who works in a tea shop earning only 9,000 taka per month. Together they live with Aliullah’s elderly parents, Abdul Barek Mridha (75) and Setara Begum (65), and the small income is barely enough to cover daily living costs, let alone emergency medical care. At 38+ weeks of pregnancy, Kulsum developed severe pain and reduced fetal movement, and an ultrasonogram revealed her baby in breech position with distress. Without an immediate caesarean section, both her life and her unborn child’s life are at serious risk. This struggling family cannot afford the surgery costs, though it is their only hope of saving mother and child.

After experiencing,lower abdominal pain for 2 days and less fetal movement for 1 day over 38+ weeks of her pregnancy , she arrived at our hospital at 11.0AM in 29 august, where doctors performed an ultrasound and clinical examination that revealed breech presentation with fetal distress , making vaginal delivery impossible. An emergency cesarean section was promptly arranged, and the procedure was successful, resulting in the birth of a beautiful female baby . Both the mother and the baby are safe and sound, with no complications arising from the surgery. We discharged the patient at 10.30AM in 01 September 2025.The patient's family expressed their heartfelt gratitude to the Helpster Heroes and the hospital for their timely and effective care.

Given the patient's condition, the doctor's prognosis emphasizes the necessity of close monitoring and timely intervention to ensure a safe outcome. If the breech presentation is left unaddressed, it may lead to fetal distress,shoulder dystocia , brain damage, or even stillbirth of the baby as well as severe perianal tear, excessive bleeding of mother or even death of mother. The doctor's treatment plan will likely involve careful fetal monitoring and a decision on the safest time and method of delivery, which will likely be a planned cesarean section. By proceeding with a medically supervised delivery, the doctors can mitigate the risks associated with both the low amniotic fluid and the prior surgical history, leading to a much safer and healthier outcome for both the mother and the baby.
