Severe malaria is a life-threatening, complicated form of malaria that progresses rapidly and is often caused by the P. falciparum parasite, characterized by serious organ failure, metabolic abnormalities, and symptoms like coma, acute kidney injury, severe anemia, and respiratory distress and Severe pneumonia is a serious lung infection, caused by bacteria, viruses, or fungi, that can lead to difficulty breathing and a lack of oxygen.

Background
Wilkista is the second-born in her family and lives with her mother in a rural village where they often struggle to get enough food. Her older siblings frequently miss school because they are sent home for lack of school fees. Despite these challenges, Wilkista is an active and curious child who enjoys scribbling whenever she sees her siblings reading or writing. Her mother, accompanied by the village administrator, came to the health facility seeking help from the hospital management due to financial difficulties. They were then referred to the Helpster coordinator for assessment. After the assessment, Wilkista was enrolled and allowed to begin treatment. Her mother reported that she had been experiencing difficulty in breathing, persistent cough, vomiting everything she ate, poor feeding, and general body weakness. On examination and laboratory investigations, Wilkista was diagnosed with severe pneumonia and severe malaria.
Medical history
Wilkista was brought to the facility in a seriously ill condition, presenting with severe malaria and severe pneumonia. She was urgently admitted and started on IV hydrocortisone, adrenaline, benzyl penicillin (X-pen), gentamicin, IV artesunate, and IV paracetamol, alongside close monitoring and supportive care. Over the course of treatment, her fever gradually subsided, her breathing improved, and she regained strength. She responded well to the medications and continued to improve steadily until she fully recovered and was discharged in stable condition.
Prognosis
Severe malaria and severe pneumonia is a serious complications especially under five years children it is an emergency and needs agent treatment and if left untreated or on late intervention it could have been resulted to some complications; acute respiratory distress syndrome (ARDS), respiratory failure, sepsis, lung abscesses, and pleural effusion, organ failure (kidneys, liver), cerebral malaria (leading to coma, seizures, or mental changes), severe anemia, respiratory distress (pulmonary edema), circulatory collapse (shock), metabolic disturbances like acidosis, and disseminated intravascular coagulation (DIC).
