The situation is critical due to the presence of severe oligohydramnios, a dangerously low level of amniotic fluid that can lead to umbilical cord compression and fetal distress. Scar tenderness is a serious risk factor, if not treated immediately, it may cause uterine rupture and patient death.This combination of factors places both the mother and the baby in a precarious position, requiring immediate and careful medical intervention.

Background
Ruksana Akter, a 24-year-old housewife with a passion for cooking, dreams of a happy life with her husband Robiul and their six-year-old son Rafi. The family lives on Robiul’s meager monthly income of 8,000 BDT from river fishing, making it nearly impossible to cover urgent medical expenses. At 37+ weeks of her second pregnancy, Ruksana developed lower abdominal pain and scar tenderness, and an ultrasound revealed severe oligohydramnios, putting both her and the unborn baby at serious risk. The doctor recommended an immediate cesarean section to save their lives, but the family’s financial constraints make this life-saving intervention difficult. Without timely surgery, both mother and child could face life-threatening complications, making urgent care critical for their survival.
Medical history
After experiencing, scar tenderness for 2 days and lower abdominal pain for 5 days over 37+weeks of her pregnancy , she arrived at our hospital at 1.00PM in 16th September 2025, where doctors performed an ultrasound and clinical examination that revealed severe oligohydro amnions with fetal distress , making vaginal delivery impossible. An emergency cesarean section was promptly arranged, and the procedure was successful, resulting in the birth of a beautiful male baby . Both the mother and the baby are safe and sound, with no complications arising from the surgery. We discharged the patient at 10.30AM in 19th September 2025.The patient's family expressed their heartfelt gratitude to the Helpster Heroes and the hospital for their timely and effective care.
Prognosis
Based on the critical diagnosis, the doctor's prognosis is that an immediate cesarean section is a life-saving intervention for both the mother and baby. The patient's previous history of caesarean section , combined with scar tenderness and severe oligohydramnios, indicate that the baby and mother are in immediate danger. If left untreated, the lack of amniotic fluid will likely lead to umbilical cord compression, cutting off the baby's oxygen and nutrient supply and potentially resulting in irreversible brain damage or even stillbirth. If scar tenderness left untreated, it may cause uterine rupture at any time resulting in death of mother and upcoming baby. Therefore, the doctor's recommendation of an immediate C-section is a crucial, time-sensitive measure to ensure a healthy and safe delivery, providing the best possible outcome for the baby's future.
