The patient is 37+ weeks pregnant with her 2nd child and has a history of a previous cesarean section, which already complicates the delivery plan. The situation is critical due to the presence of severe oligohydramnios, a dangerously low level of amniotic fluid that can lead to umbilical cord compression and fetal distress. Scar tenderness is a serious risk factor, if not treated immediately, it may cause uterine rupture and patient death.This combination of factors places both the mother and the baby in a precarious position, requiring immediate and careful medical intervention.

Background
Salma Begum, a 24-year-old housewife with a love for cooking, dreams of a happy life with her husband Jafor, their daughter Jabira, and extended family, including her disabled sister-in-law Mumtaz and elderly parents-in-law. The family survives on Jafor’s meager monthly income of 9,000 BDT as a daily wage laborer, making it extremely difficult to afford urgent medical treatment. At 37+ weeks of her second pregnancy, Salma experienced lower abdominal pain and scar tenderness, prompting her family to seek immediate medical care. An ultrasound revealed severe oligohydramnios, and the doctor recommended an urgent cesarean section to save both her and the baby’s lives. Without timely surgery, Salma and her unborn child could face life-threatening complications, making urgent intervention critical for their safety and future.
Medical history
After experiencing, lower abdominal pain for 5 days and scar tenderness for 2 days over 37+weeks of her pregnancy , she arrived at our hospital at 9.0 AM in 19th September 2025, where doctors performed an ultrasound and clinical examination that revealed severe oligohydro amnions with fetal distress, making vaginal delivery impossible. An emergency cesarean section was promptly arranged, and the procedure was successful, resulting in the birth of a beautiful female baby . Both the mother and the baby are safe and sound, with no complications arising from the surgery. We discharged the patient at 10.30AM in 22th September 2025.The patient's family expressed their heartfelt gratitude to the Helpster Heroes and the hospital for their timely and effective care.
Prognosis
Based on the critical diagnosis, the doctor's prognosis is that an immediate cesarean section is a life-saving intervention for both the mother and baby. The patient's previous history of caesarean section , combined with scar tenderness and severe oligohydramnios, indicate that the baby and mother are in immediate danger. If left untreated, the lack of amniotic fluid will likely lead to umbilical cord compression, cutting off the baby's oxygen and nutrient supply and potentially resulting in irreversible brain damage or even stillbirth. If scar tenderness left untreated, it may cause uterine rupture at any time resulting in death of mother and upcoming baby. Therefore, the doctor's recommendation of an immediate C-section is a crucial, time-sensitive measure to ensure a healthy and safe delivery, providing the best possible outcome for the baby's future.
