The patient is a 37-week pregnant woman, Gravida 4 Para 3, who presented with Premature Rupture of Membranes (PROM) — a condition where the water bag ruptures before the onset of labor. She was also found to have severe oligohydramnios, meaning a very low level of amniotic fluid around the baby, which can affect fetal movement and oxygen supply. Due to these complications, the fetus developed fetal distress, indicating signs of inadequate oxygen or compromised well-being, requiring immediate medical intervention to ensure the safety of both mother and baby.

Background
Shirina Akter, a 37-year-old kind-hearted housewife, lives a simple life in a small village with her husband Kabir Mollik, who runs a small tea shop earning only 10,000 BDT per month to support their six-member family. Despite their struggles, Shirina always dreamed of a healthy life with her husband and children — Rukaiya (19), Abir (12), and Atiq (10). During her fourth pregnancy, she developed serious complications at 37 weeks — her water broke early, and for four days she had leaking membranes with very little fetal movement. Doctors diagnosed her with severe oligohydramnios and fetal distress, warning that without an immediate cesarean section, her unborn baby could lose its life. Unfortunately, the family’s limited income makes it impossible to afford the urgent surgery, leaving them in desperate need of help to save both mother and child.
Medical history
After experiencing, passage of gush of water and leaky membrane for 4 days and less fetal movement for 2 days without labour pain, she arrived at our hospital at 1.00PM in 23 September 2025 and doctors performed an ultrasound that revealed amniotic fluid is very low with fetal distress , making vaginal delivery impossible. An emergency cesarean section was promptly arranged, and the procedure was successful, resulting in the birth of a beautiful male baby. Both the mother and the baby are safe and sound, with no complications arising from the surgery. We discharged the patient at 10.30AM in 26 September 2025.The patient's family expressed their heartfelt gratitude to the Helpster Heroes and the hospital for their timely and effective care.
Prognosis
The recommended treatment for this case is Lower Uterine Cesarean Section (LUCS), which is the safest option to deliver the baby immediately and prevent further complications. This procedure will help relieve fetal distress and reduce the risks caused by oligohydramnios and PROM, ensuring the best possible outcome for both mother and child. With timely surgery, the baby is expected to recover well and lead a healthy life. However, if the delivery is delayed or left untreated, the baby may suffer from severe oxygen deprivation, leading to permanent brain damage or even death, and the mother may also face life-threatening infections or complications.
