Usually, when this happens, hospitals put the bill on our credit until the funds are transferred to their accounts. That’s why the chronology can sometimes be in disorder.

Severe pneumonia is an advanced stage of lung infection that occurs when vital organs like the heart or kidneys are at risk, or the lungs cannot absorb enough oxygen. It often requires hospitalization, intravenous antibiotics, and may necessitate intensive care, such as mechanical ventilation, to assist with breathing. Characteristics of severe pneumonia Organ and system risk: Vital organs or systems may be at risk of failing while protein–energy malnutrition, is a form of malnutrition that is defined as a range of conditions arising from coincident lack of dietary protein and/or energy in varying proportions. The condition has mild, moderate, and severe degrees.

Background
Simon lives with his mother and extended family in the interior part of Mt. Elgon Forest. The mother is the sole breadwinner, working as a casual laborer earning about KSh 100 per day, though she often goes without work. Her income is insufficient to meet even their basic needs, and the family typically manages only one meal a day. They reside in a small two-room mud house with a rusted iron-sheet roof. The family relies on stream water for both drinking and domestic use and shares a pit latrine with neighbors and passersby, as their home is located along the roadside. Simon was brought to the hospital by the mother with complaints of a persistent cough, difficulty in breathing, and high fever. Upon examination and laboratory tests, he was diagnosed with severe pneumonia. The mother reported that the illness had been worsening gradually due to her inability to afford medical care earlier. He was admitted for further evaluation, treatment, and management.
Medical history
The mother brought her child Simon to the facility with a long history of persistent cough, difficulty breathing, and high fever, as financial challenges had delayed seeking medical care. On arrival, the child was assessed and underwent laboratory investigations. Based on the results, the child was diagnosed with severe pneumonia and started on IV ceftriaxone, IV gentamicin, IV penicillin, oral paracetamol, and other supportive treatment. The child responded gradually to treatment and eventually recovered and was discharged in stable condition. Simon was due for review a week after discharge, just to monitor his progress, of which he came to the hospital and was confirmed to be progressing well
Prognosis
With timely and appropriate medical intervention, Simon’s condition is expected to improve gradually. Treatment for severe pneumonia, combined with nutritional rehabilitation to address severe malnutrition, should stabilize his health over time. However, the child remains at high risk for complications due to weakened immunity and poor nutritional status, which may slow recovery and increase vulnerability to future infections.
