Following a detailed clinical evaluation, Rukaiya was found to have several interconnected ENT and maxillofacial conditions. She suffers from adenotonsillitis with otitis media with effusion, where inflammation of the tonsils and adenoids has caused fluid buildup in her middle ear, leading to ear blockage and hearing problems. A deviated nasal septum with hypertrophy of the inferior turbinates has been contributing to her persistent nasal obstruction and breathing difficulty. In addition, temporomandibular joint dysfunction in both ears causes pain and discomfort around the jaw and ears, while chronic maxillary sinusitis leads to facial pain, pressure, and recurrent headaches. Together, these conditions explain her ongoing symptoms and have greatly affected her daily life and well-being.

Background

Rukaiya Binte Moslema, a bright and determined 13-year-old girl, dreams of becoming a teacher one day despite her ongoing health struggles. Her father, a poor farmer, works tirelessly to support Rukaiya, her mother, and her two younger siblings, but his meager income barely covers daily necessities, leaving no means to afford costly treatment in Dhaka. As her illness worsens, Rukaiya often misses school, struggling to hear lessons and sleep at night due to constant pain and congestion. Her parents watch helplessly as her once cheerful spirit fades, fearing that without treatment, she may lose her hearing permanently and fall behind in her studies. Yet, Rukaiya remains hopeful, holding tightly to her dream of recovery and returning to the classroom with a smile.

Medical history

Rukaiya Binte Muslema was admitted to the hospital on October 30, 2025, with Adeno- tonsilitis with Otitis with Effusion (OME) and Temporomandibular Joint Dysfunctions (TMJD) with Bilateral Ears ( B/E). Her first surgery was conducted on October 30, 2025. After two days, she was discharged on October 31, 2025, in good health. Her father and mother came to the hospital to receive her. The family expressed their gratitude to Helpster, Jumuna Clinic and, Md Abdullah Miah for their treatment and support. Her mother was especially thankful for the financial assistance provided by Helpster. Her 2nd and 3rd surgeries will be conducted according to treatment plan. Rukaiya Binte Muslema was admitted to the hospital on January 15, 2026, with Adeno- tonsilitis with Otitis with Effusion (OME) and Temporomandibular Joint Dysfunctions (TMJD) with Bilateral Ears ( B/E). Her second surgery was conducted on January 15, 2026. After two days, she was discharged on January 17. 2026, in good health.

Prognosis

The doctor anticipates that the planned three-stage surgical treatment will bring significant improvement in Rukaiya’s breathing, hearing, and overall quality of life. The first surgery, adenotonsillectomy with myringotomy, will address chronic throat and ear infections, helping restore normal hearing and reduce ear discomfort. The second procedure, septoplasty with submucous diathermy (SMD), will correct nasal blockage and improve airflow, while the final functional endoscopic sinus surgery (FESS) will clear her sinuses and reduce facial pain and headaches. If left untreated, her condition could worsen over time, leading to chronic ear infections with possible hearing loss, recurrent sinus infections, sleep disturbances, breathing difficulties, speech problems, and impaired growth and learning due to poor oxygenation and repeated illness. Early surgical intervention is therefore crucial to prevent long-term complications and improve her overall well-being.
