Sickle cell anemia is a genetic blood disorder where red blood cells, normally round and flexible, become hard, sticky, and sickle-shaped (like a crescent moon) due to faulty hemoglobin, hindering oxygen transport and clogging blood vessels, causing severe pain, chronic anemia, infections, organ damage, and other serious complications, requiring lifelong management. An upper respiratory tract infection (URTI) is a common, contagious infection affecting the nose, throat, pharynx, larynx, and bronchi, typically caused by viruses (like the common cold or rhinovirus). Symptoms include nasal congestion, sore throat, cough, fever, and fatigue. The URTI is characterized by symptoms such as nasal congestion, sore throat, and cough, requiring short-term symptomatic management. The underlying sickle cell condition, associated with chronic anemia and increased susceptibility to infections, necessitates close monitoring to prevent complications during the acute illness.

Background
Joseph Adosu is a 3 year old child living in Makoko , a very impoverished area in Lagos state Nigeria. During the Meyiwavera medical outreach program, conducted in partnership with Helpster Charity in Makoko, children were being screened and Joseph was one of those children. This type of community health initiative is crucial in underserved areas, as it can identify children who might otherwise go undiagnosed and untreated. Joseph Adosu was presented at our facility with complaint of recurrent bone pain across all joints . There is also history of yellowness of the eyes, fever and passage of dark coloured urine . Upon presentation at our hospital , and after further clinical evaluation, he was diagnosed with Sickle cell Anemia and Acute Upper respiratory tract infection.
Medical history
Joseph is a 3-year-old boy with newly diagnosed HbSS who presented with recurrent bone pains across all joints, alongside a history of fever and dark-coloured urine. He was seen at Atlantis Pediatric Hospital and commenced on Folic Acid, Proguanil, Vitamin B Complex, Cetirizine, and Saline Nasal Drops. He is currently being monitored and supported through routine care.
Prognosis
The patient has a known history of Sickle Cell Anemia (HbSS) and is presently diagnosed with an Acute Upper Respiratory Tract Infection (URTI). Joseph needs monitoring for anemia and his viral infection. He has sickle cell disease with low hemoglobin (Hb 7.16) and a mild viral upper respiratory infection, but his examination and urine test are otherwise normal. Prognosis is not always good for SCD but with consistent medication, good hydration, and follow-up for his anemia, he is expected to improve.
