Why is treatment sometimes done earlier than the payment?
When a case is urgent and vital, hospitals often proceed with delivering medical care right away to save a life, even before the finalisation of admission procedures. Usually, when this happens, hospitals put the bill on our credit until the funds are transferred to their accounts. That’s why the chronology can sometimes be in disorder.

Malaria is a disease condition caused by the parasite plasmodium spp and transmitted by the vector female anopheles mosquito. It is a disease usually seen in the tropical areas. It usually presents with symptoms of fever, headache, body pains, vomiting etc. In some instances it presents as a severe disease where it causes various forms of life threatening systemic conditions like convulsions, loss of consciousness etc. Severe malaria presenting as Cerebral malaria has a similar presentation as Acute Bacterial Meningitis.

Background
Hauwa is a young girl who likes playing with her friends. She was recently enrolled in school. Due to an ongoing issue between her parents, she is now being taken care of by her step mother who is not gainfully employed. Hauwa was apparently well until few days ago when she developed fever, headache, generalized body weakness and had several episodes of convulsion. Upon presentation at our hospital, and after further evaluation, she was diagnosed with Severe Malaria (Anemia + Prostration + Convulsion) R/O Acute Bacterial Meningitis.
Medical history
Hauwa was admitted in our facility and managed as a case of Severe Malaria (Prostration + Anemia + Mild Anemia). She had her complete dosages of IV and Oral antimalarial and IV analgesics. She was also re-hydrated with IV fluids. As the treatment progressed, she responded well as evidenced by resolving of admitting symptoms. She was later discharged home with oral medications following the improvement of her condition.
Prognosis
Severe Malaria is an emergency condition which requires urgent treatment with parenteral Antimalarials and other life saving interventions as the presentation dictates. With timely and appropriate intervention, this patient has a chance of recovery, however, delay in treatment or inappropriate treatment can lead to detrimental outcomes like the loss of her life.
