Background
Ezekiel is a three-year-old boy who has not yet started school. He lives with his family. According to his mother, he is an active child who enjoys playing with toys. The family lacks a stable home and depends on the support of well-wishers. His mother reports that Ezekiel had been experiencing fever, vomiting, and body aches for three days. When his condition did not improve, she brought him to the hospital on the fourth day for evaluation and treatment. A malaria test was done and returned positive, and he was subsequently admitted to the ward with a diagnosis of severe malaria and sepsis, for further management.
Medical history
While in the ward, Ezekiel was given IV ARTESUNATE 42mg AT 0HRS,12 HRS AND 24HRS. HE was also given P.O Paracetamol syrup 5mls three times a day for three days,P.O CTX syrup 5mls twice days for three days and AL (i) twice day for three days He was later discharged on Tropical hydrocortisone cream to be applied twice a day for five days A follow up phone call confirmed that he is doing well
Prognosis
With prompt hospital admission and appropriate treatment, Ezekiel’s prognosis is generally good, despite the seriousness of both severe malaria and sepsis. Early initiation of intravenous antimalarials, antibiotics, fluids, and close monitoring greatly increases the chances of full recovery. Ezekiel is expected to respond well to treatment, regain his strength, and return to his usual playful self once the malaria parasites are cleared and the infection causing sepsis is controlled. Children often recover quickly when managed early in a hospital setting.
