This is a condition where the fetus is positioned with its buttocks or feet first instead of the usual head-down position. Due to these complications, the fetus developed obstructed labor, fetal distress, and intrauterine fetal death (IUFD).

Background
Most. Urmi is a caring and hardworking mother who looks after her young son, her sick father-in-law, and the household, while quietly dreaming of starting a small cow farm to support her family. Her husband, Younus Ali, earns only 6,000 taka a month as a day laborer, which must cover the needs of five family members living in a fragile mud house in a government housing area. After a previous caesarean delivery and with no savings, the family cannot afford sudden medical expenses. Now, at 38 weeks, Urmi is anemic and the baby is in a breech position, requiring an urgent caesarean section. Without this treatment, both Urmi and her unborn child face serious, life-threatening risks, leaving the family in fear of another tragedy.
Medical history
Urmi, a 25-year-old woman, was admitted to the hospital on 6.11.2025 at 38 weeks and 3 days of pregnancy with a breech presentation. After thorough evaluation, doctors decided to perform a Lower Uterine Caesarean Section (LUCS) to ensure the safety of both mother and baby. The operation was successfully completed. Urmi received proper post-operative care, including medication, monitoring, and nursing support, which helped her recover well. Both mother and baby remained stable throughout the recovery period, and Urmi was discharged in good health on 09.11.2025.
Prognosis
The recommended treatment for this case is a Lower Uterine Caesarean Section, which is the safest option to deliver the baby immediately and prevent further complications. This procedure will help relieve fetal distress and reduce the risks caused by the breech presentation, ensuring the best possible outcome for both mother and baby. With timely surgery, the baby is expected to recover well and lead a healthy life. However, if the delivery is delayed or left untreated, the baby may suffer from obstructed labor, fetal distress, or intrauterine fetal death (IUFD), and the mother may also face life-threatening infections or complications such as postpartum hemorrhage.
