About post-burns syndactyly with contracture

Post-burns syndactyly with contracture occurs when adjacent fingers fuse due to scar formation after thermal injury, leading to web space obliteration and restricted movement. It results from deep burns, inadequate early physiotherapy, or improper grafting. Functional and cosmetic deformities develop, impairing grasp and hand dexterity. Management involves surgical release of the web space, Z-plasty, skin grafting, or flap coverage to restore separation and mobility. Early physiotherapy, splinting, and scar management are essential to prevent recurrence and optimize function.

Abdulmumin is an 8 years old boy. He is living with his parents, currently in a government primary school. He wishes to study and become a Doctor. Parents are struggling to provide for him and the rest of his 9 siblings. Some of his elderly siblings are into menial jobs to assist in the house though not getting the jobs often. Abdulmumin is an 8-year old school boy who presented with fusion of the digits of the right hand following hot water burns 5 years ago, he was managed at home locally and discovered afterwards to have limitations in using the hand due to post-burns syndactyly of the 4 digits and contracture. Upon presentation at our hospital, and after further evaluation, he was diagnosed with Post-burns syndactyly with contracture. The contractures causes functional limitation and gross inability to perform crucial functions that every other 8 year old child can perform. The surgery can change his quality of life and improve his functional use of the hand following the burns damage.

Medical history

Abdulmumin is an 8year old school boy who presented with fusion of the digits of the right hand following hot water burns 5 years ago, he was managed at home locally and discovered afterwards to have limitations in using the hand due to post-burns syndactyly of the 4 digits and contracture. He had a contracture release surgery with full thickness skin grafting and K wire insertion, and showed good post operative outcome after 14 days. He was discharged home. The family are happy and sends gratitude for the medical support.

Prognosis

In doctor's opinion, the contractures causes functional limitation and gross inability to perform crucial functions that every other 8 year old child can perform. The surgery can change his quality of life and improve his functional use of the hand following the burns damage. The prognosis of post-burns syndactyly with contracture depends on the depth of injury, timing of intervention, and quality of reconstruction. With proper surgical release, grafting, and rehabilitation, good functional and cosmetic outcomes are achievable. However, recurrence is common, especially in children or if scar control is inadequate. Early physiotherapy, splinting, and regular follow-up greatly improve long-term results. Persistent stiffness, pigment mismatch, and hypertrophic scarring may remain, but overall hand function can be significantly restored with meticulous management and patient compliance.
