Chepkwemoi comes from a family of nine living in very difficult socio-economic circumstances. Their income is irregular and extremely low, making it hard to meet basic needs such as food, clothing, and school expenses. The family lives in a small, overcrowded house with limited access to clean water and proper sanitation. Because of these financial challenges, they often delay seeking medical care, rely on home remedies, and struggle to afford transport, consultations, and medication - putting them at greater risk of preventable health complications. Chepkwemoi arrived at the facility in labour pains, accompanied by her mother, with complaints of severe lower abdominal pain radiating to her back, increasing contractions that were unresponsive to pain medication, a headache, and fluid leakage for two days. Examination revealed a malpresentation. Malpresentation refers to any fetal position during labor that is not the normal, ideal position for birth. Normally, a baby should be in cephalic presentation, meaning the head is down and is the first part to enter the birth canal but in malpresentation the presentation is not cephalic. In this case, it was a breech presentation. Attempts by the medical team to reposition the baby and maneuver the delivery were unsuccessful, and an emergency caesarean section was recommended to prevent serious complications.

Chepkwemoi came to the facility in labour with complains of; severe lower abdominal pain radiating to the back increasing in frequency and intensity not responding to the analgesics, severe headache, drainage of the liquid and p.v bleeding. On examination she was diagnosed with labour pain in malpresentation, medical team tried to repositioned the baby but the condition gotten worsened as a result of that the professional team recommended for emergency ceaserian section to avoid high of complications to occur. The operation was successful both mother and the baby were in good condition and discharged when stable.

Chepkwemoi’s prognosis after undergoing an emergency caesarean section for breech malpresentation will be generally good, especially since the operation was performed promptly and complications were prevented early. With proper post-operative care - including wound care, pain management, monitoring for infection, and adequate nutrition - Chepkwemoi is expected to heal well and regain her strength. The baby’s prognosis is also favorable.
