Why is treatment sometimes done earlier than the payment?

When a case is urgent and vital, hospitals often proceed with delivering medical care right away to save a life, even before the finalisation of admission procedures. Usually, when this happens, hospitals put the bill on our credit until the funds are transferred to their accounts. That’s why the chronology can sometimes be in disorder.

Pyelonephritis is the medical term for a kidney infection, a serious type of urinary tract infection (UTI) where bacteria, usually from the bladder, travel up to infect one or both kidneys, causing inflammation (renal parenchyma) and the renal pelvis. Pelvic inflammatory disease is an infection of the female reproductive organs, usually caused by bacteria ascending from the lower genital tract. It may present with lower abdominal pain, fever, abnormal discharge, painful urination, and can lead to complications if not treated early.

Background

Aisha comes from an extremely underprivileged background marked by persistent financial hardship. Her family has no stable source of income and struggles daily to afford basic necessities such as food, clean water, and safe shelter. Because of their economic situation, they are unable to access basic healthcare services, including medical consultations and prescribed medication. As a result, most illnesses are either untreated or managed at home with limited resources. The family lives in an area with poor sanitation, characterized by overcrowding, unsafe drinking water, and inadequate waste disposal. These conditions expose Aisha and her household to frequent infections, contributing to repeated illness, poor overall health, and increased vulnerability to preventable diseases. Aisha presented to the hospital with sudden onset flank pain associated with intermittent chills that gradually worsened and did not respond to over-the-counter medication. She had also previously visited small local clinic located in a very remote area, with limited capacity and minimal equipment to manage serious medical conditions. The services at this clinic are largely restricted to dispensing basic, low-cost oral medications, which are often inadequate for managing severe illnesses. Due to the long distance to better-equipped health facilities, at times families are compelled to seek care at this clinic despite its limitations. Aisha was clearly in pain. She also reported painful urination, swelling of the lower limbs, and dark-colored urine. As her condition deteriorated and the family realized they could not afford hospital care, a neighbour who had previously benefited from Helpster Charity advised them to seek care at the facility and informed them about the organization’s support for underprivileged patients. On examination and laboratory investigations, Aisha was diagnosed with pyelonephritis and pelvic inflammatory disease. She was admitted for treatment and further medical management.

Medical history

Aisha came in the facility with complains of; sudden onset of flank pains associated with Chills on and off worsening gradually not responding to OTC drugs,she had developed pain during urination, lower limb swelling and dark coloured urine. On examination and laboratory findings she was diagnosed with nephritis, pyelonephritis and pelvic inflammatory disease. Was admitted for several days until she stabilized and discharged on good condition.

Prognosis

With timely hospital admission and appropriate treatment, Aisha’s prognosis is good. Both pyelonephritis and pelvic inflammatory disease respond well to the correct antibiotics and supportive care when managed early. As the infection clears, her pain, fever, and urinary symptoms are expected to resolve. Completing the full course of treatment and attending follow-up visits will be important to prevent recurrence or long-term complications.
