A severe UTI with haematuria indicates a more aggressive or advanced infection that may involve the bladder (severe cystitis) or upper urinary tract, and it carries a higher risk of complications such as kidney involvement, sepsis, or ongoing bleeding if not treated promptly.

Background

Daniel is a 6-year-old boy living at a Children’s Home in Pipeline, Nairobi. He is an orphan who enjoys music and playing with the other children. The Home relies entirely on well-wishers for food, shelter, and the children’s overall care. Daniel was taken into the Home five months ago after his single mother was tragically killed by thugs while returning home from work. Efforts to trace any remaining relatives are still ongoing. Despite the challenges, Daniel dreams of becoming a teacher one day. The Children’s Home learned about Helpster Charity after a site visit by Rheemah medical staff and later through Vincent, a Helpster volunteer. Daniel had been well until five days prior, when he developed blood-stained urine, lower abdominal pain, frequent and painful urination, and fever. Concerned about his condition, Sarah, a volunteer, brought him to Rheemah Hospital for medical attention. He was examined by the medical officer, and following a comprehensive clinical evaluation and appropriate investigations, Daniel was diagnosed with a severe urinary tract infection (UTI). Given the history of haematuria (blood-stained urine) for five days, further diagnostic tests were conducted to rule out schistosomiasis and other possible causes of hematuria. Once these conditions were excluded, treatment for the severe UTI was initiated promptly. Daniel was managed as an outpatient but required daily intravenous medication administered once daily for three consecutive days. The first dose was given on the day of presentation, and he was scheduled to return to the hospital on the following two days to complete the course of injectable treatment.

Medical history

Daniel Nzioka has been well until 4 days ago where he started experiencing Blood in urine,LAPs, Frequent Urination, painful urination and Fever. Sarah who is a volunteer brought him to Rheemah Hospital for Medical assistance. Daniel was seen by medical officer in charge and after a thorough Examination and test done he was diagnosed with UTI . Prompt treatment began and he was put on i.v ceftriaxon 1g, i.v Gentamycin 80mg, Proximexa 250mg srp and Brustan srp . He is now doing well and back to his joyful self again.

Prognosis

With the diagnosis clearly established and appropriate treatment initiated promptly, Daniel’s prognosis is good. The structured course of intravenous therapy, along with close outpatient follow-up, is expected to effectively resolve the infection and alleviate his symptoms. Since other serious causes of blood-stained urine were ruled out, and provided he completes the full treatment regimen and attends all follow-up visits, Daniel is expected to make a full recovery without long-term complications.
