Fetal distress, also known as Non-reassuring fetal heart rate (FHR) refers to an abnormal fetal heart rate pattern that suggests the baby may not be getting enough oxygen in the womb. It includes findings such as persistently very fast or very slow heart rates, reduced variability, or repeated decelerations, and indicates possible fetal distress requiring urgent evaluation and intervention. Meconium staining of the liquor is also a sign of fetal distress. Severe anaemia in pregnancy is a condition in which a pregnant woman has a markedly low hemoglobin level - usually below 7 g/dL - meaning her blood cannot carry enough oxygen to meet the needs of both the mother and the unborn baby. It increases the risk of maternal complications, poor fetal growth, preterm delivery, and fetal distress.

Background

Chemtai comes from a family experiencing severe socio-economic hardship. The household depends on irregular, poorly paid casual labor that is often insufficient to meet even their most basic needs. With no stable income, the family struggles to afford adequate food, clothing, proper housing, and access to clean drinking water. As a result, essential services such as healthcare is difficult to afford. Education has also been greatly affected, as the family cannot consistently afford school fees, uniforms, books, or related expenses, leading to repeated interruptions in schooling despite the children’s willingness to learn. Chemtai presented to the health facility in active labor, accompanied by her mother. She complained of severe lower abdominal pain associated with drainage of fluid, with the pain increasing in frequency and intensity and not responding to analgesics. She also reported headache and dizziness. One week prior, she had been treated for a urinary tract infection. On examination and laboratory evaluation, Chemtai was found to have cervical dilation of 6 cm with no progress over a prolonged period, grade three meconium-stained liquor, absence of fetal descent, and a hemoglobin level of 6.7 g/dL. She was diagnosed with prolonged labor complicated by fetal distress and severe anemia. Given the high risk to both mother and baby, the medical team recommended an immediate emergency cesarean section to save their lives and prevent further complications.

Medical history

Chemtai came in the facility in labour accompanied by her mother with complains of severe abdominal pain with drainage of liquid, pain reported to be increasing in frequency and intensity not responding to analgesic, headache and dizziness. On examination and laboratory investigation, she was 6 cm cervical dilation without any progress for a long time with Meconium grade 3 and Hb of 6.7mm/L . She was diagnosed with fetal distress due to prolonged labour and anemia. The Medical team recommended for emergency ceaserian section to be contacted immediately for the purpose of saving lives as the only safest method, operation was successful both mother and the baby were in good condition and discharged when stable.

Prognosis

With timely intervention through an emergency cesarean section, close monitoring, and appropriate management of her severe anemia, Chemtai’s prognosis is hopeful. Prompt surgical delivery significantly reduces the risk of life-threatening complications such as uterine rupture, severe hemorrhage, infection, and fetal loss. Chemtai received adequate perioperative care, including blood transfusion and proper postnatal follow-up. She is expected to recover well. The baby’s outcome is good since delivery was achieved quickly and we are hopeful that there will be good neonatal care provided. Early intervention greatly improves the chances of survival and recovery for both mother and child, and the prognosis for both mother and baby is generally good.
