Severe Dehydration: A serious condition where the body loses a critical amount of fluids and electrolytes, due to diarrhoea and vomiting in this case, often causing weakness, low blood pressure, rapid heartbeat, and in severe cases, organ failure if untreated. Peptic Ulcer Disease (PUD): A condition where open sores develop on the lining of the stomach or the first part of the small intestine, usually due to H. pylori infection or prolonged NSAID use, causing pain, indigestion, and complications if untreated. Vomiting blood in a patient with PUD indicates upper gastrointestinal bleeding, usually from a bleeding ulcer, which is a medical emergency requiring prompt treatment to prevent severe blood loss and complications.

Background
Agnes lives with her mother and is an only child. Agnes had been healthy until four days ago when she began experiencing vomiting of blood, passing blood-stained stool, abdominal fullness, and epigastric pain. Concerned and unable to afford medical care, her mother contacted the local Community Health Worker, Madam Vennesa, who informed Vincent, a Helpster volunteer. Through their intervention, Agnes was introduced to Helpster Charity and Rheemah Hospital, where she received assistance. Her mother hopes that Agnes will become a doctor in the future. Upon worsening of her symptoms, Agnes was brought to Rheemah Hospital for thorough examination and testing. She was diagnosed with Helicobacter pylori infection, peptic ulcer disease (PUD), and acute gastritis with bleeding. She was admitted and started on prompt treatment under continuous observation by doctors and nurses. She was severely dehydrated when she presented to the hospital, and was still vomiting profusely.
Medical history
Upon worsening of her symptoms, Agnes was brought to Rheemah Hospital for thorough examination and testing. She was diagnosed with Helicobacter pylori infection, peptic ulcer disease (PUD), and acute gastritis with bleeding. She was admitted and started on prompt treatment under continuous observation by doctors and nurses. Agnes responded well to the treatment and was discharged on the third day with instructions to rest at home and continue her oral medications. The attending doctor assured her that she would recover fully within a week.. Agnes was admitted and prompt treatment was initiated, she was put on i.v normal saline, i.v Tramadol, i.v Ondestron 8mg, i.v Esomeprazole 40mg, i.v Hyocine 20mg and Discharged on Pylotrip kit, Tramacetal tabs, Relcergel Susp with constant observation of the Doctors and Nurses on duty she responded well to treatment, before he was allowed to go home on the 3rd day . He was was advice to have a rest at home while taking the orals and the Doctor assure her she will be better in les than a week.
Prognosis
With timely medical intervention, Agnes’ prognosis is guarded but favorable. Following admission, prompt initiation of treatment for Helicobacter pylori infection, peptic ulcer disease, and acute gastritis with bleeding, along with intravenous fluids to correct severe dehydration, significantly improves her chances of recovery. Close monitoring by the medical team reduces the risk of complications such as ongoing bleeding, anemia, or shock.
