Thyroglossal cyst. A thyroglossal cyst is a congenital midline neck mass that arises from persistent remnants of the thyroglossal duct during embryonic thyroid descent. It commonly presents in children or young adults as a painless, soft, mobile swelling located near the hyoid bone. The cyst characteristically moves upward with swallowing or tongue protrusion. Infection may cause pain, redness, or drainage, and in some cases, it may form a sinus or fistula. Diagnosis is clinical and supported by ultrasound. Definitive treatment is the Sistrunk procedure, which involves excision of the cyst, tract, and central hyoid bone to prevent recurrence. Prognosis is excellent.

Ahmed an 8year old male child is the 7th child among 9 siblings in a polygamous setting. He is currently staying with his parents and undergoing his primary education. The family are staying in a 3 rooms apartment and a single toilet. The family sometimes struggle to feed as the father is not having much income from the petty trading he is doing. They make use of water from the well for their daily activities. They have don't access to electricity. He is an 8 year old school boy who presented with a progressively increasing neck swelling for about 2 years. The swelling was examined and found to be a cystic swelling moving with protrusion of the tongue which is consistent with the diagnosis of a Thyroglossal Cyst. This is causing difficulty, some airway obstruction, infection, and discomfort - important to get the surgery. He will require a Sistrunk operation to remove the cyst, dissect the tract, remove the middle part of the hyoid bone and a small part of the tongue to prevent recurrence. Kindly render the necessary assistance he would require for this surgery as the family are not financially capable to pay that is why they are here for Helpster Charity to assist.
Prognosis
Ahmed is facing major dangers of a thyroglossal cyst which include infection, which can lead to airway obstruction, difficulty swallowing, and drainage issues, and in rare cases, cancer. If the cyst becomes infected, it can rapidly enlarge and cause a serious threat to breathing, especially in children. The prognosis of a thyroglossal cyst is good, especially with early and proper surgical management. After the Sistrunk procedure—which removes the cyst, its tract, and the central portion of the hyoid bone—the recurrence rate is very low. Most patients recover fully without long-term complications. If the cyst was previously infected, antibiotics and drainage may be needed before surgery. Rare complications include recurrence, infection, or scarring. Malignant transformation is extremely uncommon but possible, typically presenting as papillary thyroid carcinoma within the cyst. With timely surgery and follow-up, patients usually experience complete resolution and return to normal activities with no functional problems.
