Mamuli Akter is 37+ weeks pregnant with her second child, and the baby is in a breech presentation, meaning the baby’s feet or buttocks are positioned to come out first instead of the head. This position makes a normal (vaginal) delivery dangerous. Additionally, Mamuli has a previous history of cesarean section due to complications in her first pregnancy, which further increases the risk of uterine rupture or distress during labor.

Background
Mamuni Akter, a gentle and hardworking 23-year-old mother, is now 37+ weeks into her high-risk second pregnancy, but her family’s financial struggles make it impossible for them to afford the treatment she urgently needs. Her husband, Md. Raju, works as a day laborer, earning barely enough to support their household of six, which includes Mamuni, their 5-year-old daughter Sadia, and her in-laws. With the baby in a dangerous breech position and a history of complications in her first pregnancy, Mamuni faces life-threatening risks if she attempts a normal delivery. Without a safe, medically supervised delivery—most likely a caesarean section—both her life and the baby’s life could be in serious danger. Proper treatment is crucial not only to protect Mamuni and her unborn child but also to prevent her young daughter and family from facing a devastating loss
Medical history
The patient, Mamuni was admitted to City Clinic on 16th November 2025. A successful cesarean section was performed the same day, and she delivered a healthy 2.4 kg female baby. Both mother and child progressed well and were discharged on 19th November 2025 with proper postnatal advice. The patient and her family expressed heartfelt gratitude to Helpster, the dedicated volunteers, and City Clinic for their kind support and care throughout the treatment.
Prognosis
The doctor has advised a Lower Uterine Cesarean Section (LUCS) as the safest treatment option for Mamuli Akter's delivery. With this procedure, both mother and baby are expected to have a stable and healthy outcome, minimizing the risks associated with breech presentation and her previous cesarean history. The surgery will help prevent life-threatening complications such as uterine rupture, fetal distress, or prolonged labor. If the condition is left untreated and a vaginal delivery is attempted, both Mamuli and her baby could face severe complications, including the risk of losing the baby or endangering the mother’s life. Timely surgical intervention is therefore critical for a safe and successful delivery.
