Kaltum Sani, an 19-year-old female, presented with lower abdominal and pelvic pain for the past 2 months. The pain is intermittent, dull, and sometimes radiates to the back, and worsens during menstruation. She reports abdominal bloating and a sense of fullness in the lower abdomen. Her menstrual cycles have recently become irregular, with heavier than usual flow. She has also experienced nausea and mild discomfort during daily activities. No fever, vomiting, or urinary symptoms are reported. There is no history of previous pelvic surgery. She has generally been healthy with no chronic illnesses. Imaging and clinical findings are consistent with a complex ovarian cyst, which is a fluid-filled sac within the ovary that may contain both cystic and solid components, sometimes with internal septations. Such cysts can cause pain, menstrual irregularities, and pressure symptoms. Surgical removal (cystectomy or oophorectomy) is planned, pending approval from the Helpster Charity Foundation, due to the cyst’s size, complexity, and persistent symptoms.

Background
Kaltum Sani is an 19-year-old young woman from a supportive and humble family. She is a diligent student who balances her studies with helping at home. Kaltum is generally healthy with no known chronic illnesses or previous surgeries. Her family describes her as responsible, calm, and kind. She has had no significant hospital admissions before. She presented at the hospital with lower abdominal and pelvic pain for the past 2 months, with attendant abdominal bloating, lower abdominal fullness, irregular and heavier menstrual cycles, nausea and mild discomfort during daily activities. Moderately well with mild comfort on general physical examination. On abdominal examination,there is presence of mild tendeerness in lower abdomen and palpable firm mass on the left adnexal region and complex ovarian cyst was diagnosed.
Medical history
Kaltum, an 19-year-old female, was referred to our facility with Complex Ovarian cyst which was also confirmed in UMD.After thorough evaluation and investigation, she underwent ovarian cystectomy. The surgery was successful without immediate complications. Due to the nature of her sickness, she was admitted for 5 days for close monitoring and treatment.She recovered well and was discharged home after 5 days, with follow-up care scheduled for 1 week. Currently, Kaltum is doing well, has recovered, and has resumed regular activities. We sincerely appreciate Helpster charity foundation for the free medical services.
Prognosis
Kaltum needs an urgent surgery due to the symptoms and to avoid complications. Complex ovarian cysts can potentially grow larger, causing increased pain, discomfort, and pressure on surrounding organs. There is also a risk of rupture, which can lead to severe pain, internal bleeding, and infection. In some cases, complex cysts may be cancerous, and delayed treatment could allow the cancer to advance to a more advanced stage, reducing the chances of successful treatment. Surgery may involve removal of the cyst (cystectomy) or, in more severe cases, removal of the entire ovary (oophorectomy) Surgery is the only option and Kaltum will significantly benefit from the procedure.
