About obstructed inguinal hernia
An obstructed inguinal hernia occurs when abdominal contents, usually intestine, become trapped in the inguinal canal, causing blockage of bowel passage without loss of blood supply. It commonly presents with a painful, irreducible groin swelling, abdominal distension, vomiting, and constipation. Risk factors include chronic cough, heavy lifting, straining, and weak abdominal muscles. The swelling is tense and tender, and bowel sounds may be exaggerated early. Unlike strangulation, vascular compromise is absent initially, but obstruction can progress to strangulation if untreated. Prompt diagnosis and surgical intervention are essential to relieve obstruction and prevent complications such as ischemia, perforation, and sepsis.

Ryan a 2yo male child is the 3rd child of his parents, he has two siblings and are all staying with their parents in a two rooms apartment in a small community in Plateau State, Nigeria. The father is struggling to provide for the family with his petty trading. The mother is a house wife. They make use of water from the well for their daily activities, they have access to electricity. Ryan a 2year old male child brought to the hospital by his mother with complaints of intermittent swelling and pain in the groin for about 6 months. Swelling was initially reducible but now associated with severe pains and not readily reducible at the moment. On examination Male toddler, crying in mother's arm, not pale not cyanosed , moves with respiration Right groin swelling with marked tenderness. Not reducible. The parents reached out to Helpster Charity through a volunteer seeking for help because they don't have the money for the bills. He was diagnosed with Obstructed inguinal hernia and causing significant pain and in danger of strangulated if not urgently handled it can lead to a worse outcome.
Prognosis
For Ryan a 2yo male child, the hernia is obstructed and causing major pain and if left can lead to strangulation. Prognosis is excellent if treated early. At his age, prompt surgical intervention usually leads to full recovery with minimal complications. If the obstruction is relieved before bowel strangulation occurs, there is low risk of long-term morbidity. Delay in treatment can worsen prognosis, leading to bowel ischemia, necrosis, or perforation, which increase morbidity and hospital stay. With timely diagnosis, skilled pediatric surgery, and proper postoperative care, Ryan will recover completely and resume normal growth and development.
