The patient is a 37 weeks and 2 days pregnant woman, gravida 4 and para 3, who presented with a breech presentation, a condition in which the fetus is positioned in the uterus with its bottom or feet pointing down instead of the usual head-down position. Due to this complication, the fetus was at risk of fetal distress, umbilical cord prolapse, birth trauma such as brachial plexus injury, and head entrapment, requiring immediate medical intervention to ensure the safety of both the mother and the baby.

Background
Anoara, a 40-year-old housewife who dreams of building a home of her own, spends her days sewing kantha while caring for her family of six. Her husband, Torikul Islam, earns only 6,000 taka a month as a rickshaw puller, and the family survives on fragile government land while supporting two school-going sons, a toddler daughter, and an elderly mother-in-law. When Anoara reached 37 weeks of pregnancy, severe abdominal pain and breathing difficulty revealed a dangerous breech presentation, making an emergency caesarean section the only option to save her baby. Without this life-saving surgery, both mother and unborn child face serious risk, including birth complications or death. With no savings and overwhelming medical costs, the family has no way to afford the treatment without support.
Medical history
Anoara was admitted with a 37 weeks and 2 days pregnancy complicated by a breech presentation on 24.10.2025. After proper evaluation, an LSCS was performed on the same day. The operation was successful, and a healthy baby was delivered safely. Following surgery, she received appropriate medical management, close monitoring, and attentive nursing care, which supported her smooth recovery. Her condition gradually improved, and both mother and baby remained stable and secure throughout the hospital stay. She was discharged in good condition on 27.10.2025.
Prognosis
The recommended treatment for her is LSCS under SAB, which is the safest option to deliver the baby immediately and prevent further complications. With timely surgery, the baby is expected to recover well and lead a healthy life. However, if the delivery is delayed or left untreated, the baby may suffer from oxygen deprivation leading to fetal distress, umbilical cord prolapse, birth trauma such as brachial plexus injury, and head entrapment, and the mother may also face life-threatening infections or complications.
