When a case is urgent and vital, hospitals often proceed with delivering medical care right away to save a life, even before the finalisation of admission procedures. Usually, when this happens, hospitals put the bill on our credit until the funds are transferred to their accounts. That’s why the chronology can sometimes be in disorder.

About infections
A Lower Respiratory Tract Infection (LRTI), or chest infection, affects the airways and lungs below the voice box, commonly including bronchitis (inflammation of bronchial tubes) and pneumonia (infection in lung tissue). Lower respiratory tract infection and severe malaria Lower respiratory tract infection is an illness of the lungs that causes cough, fever, fast or difficult breathing, and chest pain. Severe malaria is a dangerous malaria infection that leads to high fever, extreme weakness, anemia, and damage to vital organs. When these two conditions occur together, they place great stress on the body, reduce oxygen levels, and make breathing harder. This combination is very serious and requires urgent hospital treatment with antimalarial medicines, antibiotics, oxygen, and careful monitoring to prevent severe complications or death.

Background
Merabel is 7-month old female child. She is the third child of her parents. The father is not currently working as he is still struggling to get a job. The mother is a house wife. They have been in financial crisis for sometimes that is why they sought for help from Helpster Charity. They live in a 2 room apartment with a pit toilet with no access to electricity and they make use of water from the well for their daily activities. Merabel started having fever, cough, rhinorrhea, weakness and abdominal pain. On examination, a 7 months old baby girl, ill looking, tachypneic, febrile with uhtemperature 37.7 degrees Celsius, restless, uniteric and acyanosed. Patient was admitted, investigations revealed lower respiratory tract infection and severe malaria, treatment commenced immediately. The family were at the hospital but could not pay for their child's medical bill, they were placed under Helpster Charity for medical support through Helpster Charity volunteer. Diagnosis is Acute Lower Respiratory Tract Infection and Severe Malaria.
Medical history
Merabel a 7month old female child presented with the following complaints; Fever, cough, rhinorrhea, weakness and abdominal pain. On examination, she is ill looking, tachypneic, febrile with temperature 37.7 degrees Celsius, restless, uniteric and acyanosed. Patient was admitted, investigations revealed lower respiratory tract infection and severe malaria. Treatment commenced with injection antimalarials, antibiotics and other supportive care, patient did well and was stable, temperature was normalized, coughing stopped, crying stopped and feeding continued well. Patient is to return for follow up after a period of 1 week and the family remained grateful to Helpster Charity for the medical support.
Prognosis
Merabel, a 7-month-old female child with severe malaria and a lower respiratory tract infection is critically ill and requires urgent hospital care. At this age, immunity is immature, making her highly vulnerable to rapid deterioration. Severe malaria can cause anemia, low blood sugar, and organ dysfunction, while the chest infection increases breathing difficulty and reduces oxygen levels. Together, these conditions significantly raise the risk of respiratory failure and sepsis. Immediate treatment with intravenous antimalarials, appropriate antibiotics, oxygen support, careful fluid management, and close monitoring is given to improve survival and prevent long-term complications.
