About severe malaria and exacerbation of peptic ulcer disease
Severe malaria is a life-threatening parasitic infection characterized by high parasitemia, anemia, hypoglycemia, metabolic acidosis, and possible organ dysfunction. The intense systemic inflammatory response, stress, and reduced immunity associated with severe malaria can exacerbate pre-existing peptic ulcer disease (PUD). Fever, vomiting, poor oral intake, and the use of non-steroidal anti-inflammatory drugs may further worsen gastric mucosal injury. Exacerbation of PUD may present with epigastric pain, hematemesis, or melena, complicating management. Prompt antimalarial therapy, gastric acid suppression, fluid resuscitation, and careful monitoring are essential to prevent morbidity and mortality.

Background
Tabither is a 21yo young female adult. She is currently living with her parents and 5 siblings. Tabitha wishes to become a nurse because of her love to take care of the sick and needy. The parents are struggling financially to ensure that she achieves this dream. They are living in 2 rooms apartment and a pit toilet. They make use of well water for their daily activities, they also have access to electricity. Tabitha started having headache/dizziness for 2 days, fever, joint pain, vomiting everything for 2 days, insomnia, tastelessness, bitterness, abdominal pain and rhinorrhea. On examination, a 21year old female young adult looking febrile with temperature 38.5degree Celsius, weak and shivering of cold, not jaundiced, with continuous vomiting and severe abdominal pain. Patient was diagnosed with severe malaria and exacerbation of peptic ulcer disease. The family had no money to take her to the hospital, they were informed about Helpster Charity from some beneficiaries. They brought her to Albishir Hospital for proper evaluation.
Medical history
Tabitha a 21-year old presented with the following complaints; headache/dizziness for 2 days, fever, joint pain, vomiting everything for 2 days, insomnia, tastelessness, bitterness, abdominal pain and rhinorrhea. On examination, a 21year old female young adult looking febrile with temperature 38.5degree Celsius, weak and shivering of cold, not jaundiced, with continuous vomiting and severe abdominal pain. Patient was diagnosed with severe malaria and exacerbation of peptic ulcer disease. Treatment commenced with antimalarial injections, antibiotics, intravenous fluids to mitigate loss of fluids and omeprazole for the exacerbation of peptic ulcer disease and other supportive care. Patient became stable with the extensive care given, vomiting stopped, temperature was normalized, no more headache and insomnia, feeding is excellent, abdominal pain is gone as well as joint pains is gone. Patient is to return for follow up after a period of 1 week. The family gave thanks to Helpster Charity for saving their child.
Prognosis
Tabitha's condition was timely administration of effective antimalarial therapy, adequate supportive care, and appropriate management of gastric complications, she is recovering fully. However, delayed treatment may result in complications such as severe anemia, hypoglycemia, gastrointestinal bleeding, shock, or multi-organ failure, which worsen outcomes. The active ulcer bleeding or significant comorbidities have a higher risk of morbidity. Overall prognosis is good in well-equipped settings with close monitoring and early intervention. Tabitha has been stabilized and recovery is having effect.
