When a case is urgent and vital, hospitals often proceed with delivering medical care right away to save a life, even before the finalisation of admission procedures. Usually, when this happens, hospitals put the bill on our credit until the funds are transferred to their accounts. That’s why the chronology can sometimes be in disorder.

Severe malaria with febrile convulsions is a life-threatening form of malaria, most commonly caused by Plasmodium falciparum, in which the infection leads to high persistent fever accompanied by seizures (convulsions). In this condition, the malaria parasites multiply rapidly in the blood, causing intense fever and systemic illness. The very high temperature, combined with the effects of the infection on the brain and metabolism, can trigger convulsions. Severe malaria with febrile convulsions requires urgent hospital treatment, close monitoring, and prompt administration of intravenous or intramuscular antimalarial medication to prevent brain injury, long-term complications, or death.

Background
John is the second-born in a family of three children. He is currently in Grade Three at a rural junior school. At home, he enjoys playing with his peers and is described as an active and cheerful child. His mother operates a small-scale business that sustains the family, earning an average monthly profit of approximately KES 3,000. The family fetches drinking water from a borehole and lives in a modest two-room rented house, sharing a pit latrine with other tenants. John aspires to become a pilot in the future and is passionate about continuing his education. Three days prior to hospital presentation, John began experiencing persistent vomiting, high fever, and episodes of convulsions. His condition deteriorated rapidly, leaving him weak and unable to attend school. He became increasingly lethargic, with reduced appetite and minimal interaction with family members. The convulsions were particularly distressing, and his mother grew deeply concerned as his level of alertness fluctuated. Due to financial constraints, the family was unable to afford medical consultation and treatment. Fortunately, a volunteer from Helpster Charity learned of John’s condition and advised the family to take him to Mechimeru Health Centre, where he could receive medical care supported by the organization. By the time John was brought to the hospital, he appeared critically ill. He was febrile, weak, and visibly dehydrated from repeated vomiting. His responsiveness was reduced, and he required urgent medical assessment and stabilization. Immediate intervention was necessary to manage his symptoms and prevent further complications.
Medical history
John’s condition requires immediate hospital treatment. He was given intravenous antimalarial medicine (artesunate) to rapidly kill the parasites, followed by oral antimalarial drugs once his condition improves. He also receives supportive care, such as fluids, treatment for low blood sugar, blood transfusion if anemic, and close monitoring of vital of signs. With prompt and proper treatment, John is likely to recover fully. However, without early treatment, severe malaria can cause organ damage or death.
Prognosis
With prompt and appropriate treatment, severe malaria is often survivable. Most patients improve rapidly once antimalarial therapy is started, and many can return to normal or near-normal life. Recovery may take weeks, and some patients may have temporary or, in severe cases, lasting complications such as weakness, anemia, or neurological or kidney problems. If left untreated, severe malaria progresses quickly and is frequently fatal. The infection can cause coma, organ failure (brain, kidneys, lungs), severe anemia, and metabolic collapse. Death can occur within days, and survivors often suffer permanent disabilities. Overall Early treatment greatly improves survival and quality of life; lack of treatment almost always leads to severe complications or death.
