An umbilical hernia is a protrusion of the bowel or its part via a defect in the Umbilicus. Normally before birth, the umbilicus is closed. But in some cases there is persistence of the opening in the umbilicus while in some patients it is acquired, in such conditions patients end up presenting with recurrent swelling of the umbilicus which gradually increases in size and returns either spontaneously or manually unless if obstructed or strangulation.

Background
Ari is a calm 15-year-old boy who enjoys going to school and helps his father with farm work at home. His father is a small-scale farmer, while his mother sews traditional caps to support the family, and they struggle to manage with limited resources. Ari presented with an umbilical swelling present since birth, which was initially small but gradually increased in size, occasionally becoming hard and painful. He was diagnosed with an umbilical hernia and requires surgical repair to correct the defect and prevent complications in the future.
Medical history
Ari presented with umbilical swelling which father said to have been present since birth, occasionally becoming hard and painful and non-reducible. After proper evaluation, he was diagnosed with umbilical hernia. He had surgery done successfully where the defect was corrected. His postoperative period was satisfactory as no complication was noted. He was then discharged home with oral medications to complete his recovery and return to his normal premorbid condition.
Prognosis
We view Bulama's umbilical as one which needs to be surgically corrected, which is very necessary to prevent future risks like bowel entrapment. The prognosis is excellent, with the procedure usually performed as a routine day-case surgery that allows for a quick and full return to normal activity. With surgical treatment, most patients recover well. However, when left untreated there might be obstruction or strangulation of the bowel causing serious and life threatening conditions.
