Why is treatment sometimes done earlier than the payment?

About severe protein–energy malnutrition (pem):

A life-threatening condition that occurs when the body does not receive enough protein and calories for a prolonged period. It leads to severe weight loss, muscle wasting, weakened immunity, delayed growth, and increased risk of serious infections and death, especially in children. Severe Malaria: A dangerous form of malaria caused mainly by Plasmodium falciparum, in which the infection affects vital organs and causes very high fever, severe weakness, anaemia, vomiting, confusion, breathing difficulty, or inability to feed. It is a medical emergency requiring urgent hospital treatment. Tungiasis (Jigger Infestation): A parasitic skin disease caused by the sand flea that burrows into the skin, usually on the feet. Severe infestation causes pain, swelling, infection, difficulty walking, and risk of secondary bacterial infection, particularly in children living in poor sanitation conditions. Heavy Intestinal Helminth Infestation: A condition in which a child has a large number of intestinal worms, leading to abdominal pain, vomiting worms, poor appetite, malnutrition, anaemia, intestinal blockage, and severe weakness. Heavy worm infestation can become life-threatening if untreated.

Background

Bonface comes from a highly disadvantaged community where poverty is widespread and most families struggle to meet even the most basic needs. Many households rely on irregular, low-income activities, making it difficult to afford adequate food, proper housing, or healthcare. Homes are often built from mud and lack cemented floors, while access to clean water, proper sanitation, and waste disposal is very limited. These conditions create an environment where jigger (tungiasis) infestation is common, especially among children who often walk barefoot due to lack of shoes. Food insecurity is also a major challenge, with many families unable to afford balanced meals. As a result, malnutrition is widespread, leading to weakened immunity, poor growth, and increased vulnerability to infections. Limited access to health facilities, low health awareness, and inability to pay for treatment mean that illnesses are often untreated or managed very late, trapping families in a cycle of poverty, disease, and poor health. Bonface was brought to the health facility by his sister with a history of vomiting worms, abdominal pain, inability to pass stool, joint pains, loss of appetite, general body weakness, weight loss, high fever, and persistent greenish vomiting, along with jigger infestation. Following clinical examination and laboratory investigations, he was diagnosed with severe protein–energy malnutrition, dehydration, severe malaria, mild anaemia, and tungiasis. He was admitted for treatment, close monitoring, and further medical management.

Medical history

Bonface came to the facility accompanied by her sister with complains of vomiting worms, abdominal pain with not able to pass stool, joint pain, general body weakness,high fever, loss of appetite, weight loss and infestation of jiggers (tungiasis). On examination and laboratory investigation, he was diagnosed with severe protein -Energy malnutrition, dehydration, mild anemia, severe malaria and tungiasis (jiggers infestation). He was admitted for several days and responded well to medication and discharged on a good condition.

Prognosis

Doctor’s Opinion Bonface presented in a critically ill and severely weakened state with multiple serious conditions, including severe protein–energy malnutrition, severe malaria, dehydration, heavy intestinal worm infestation, mild anaemia, and tungiasis. His symptoms - persistent vomiting, inability to pass stool, weight loss, high fever, and extreme weakness - indicate advanced illness and significant risk of life-threatening complications. The combination of severe malnutrition and infection has greatly compromised his immunity, placing him at high risk of rapid deterioration without urgent and comprehensive medical care. He requires immediate inpatient treatment, nutritional rehabilitation, antimalarial therapy, deworming, wound care, and close monitoring. Prognosis With timely, intensive medical treatment and sustained nutritional support, Bonface has a chance of recovery. However, without continued care and assistance, his condition could quickly worsen and become life-threatening. Medical support, follow-up care, and social assistance are therefore critical to saving his life and restoring his health
