Hepatocellular injury is a condition in which the liver cells (called hepatocytes) are damaged or destroyed due to disease, infection, toxins, or other harmful factors. Hypokalemia is a condition with low potassium in the blood (below 3.5 mEq/L), impacting nerve, muscle, and heart function, causing symptoms like weakness, fatigue, cramps, constipation, and potentially dangerous irregular heartbeats (arrhythmias). Severe anemia means your blood has a dangerously low level of healthy red blood cells, drastically reducing its ability to carry oxygen, leading to extreme fatigue, shortness of breath, dizziness, and potentially serious complications like heart problems, affecting daily function and requiring urgent medical attention for its underlying cause. Severe malaria occurs due to delayed treatment of uncomplicated malaria and is defined by clinical or laboratory evidence of vital organ dysfunction.

Background
Linet is the last-born child in her family. She lives with her mother and two sisters in a two-room mud-walled house. Due to financial constraints, Linet does not attend school. Her mother is the sole breadwinner and relies on casual jobs to support the family. Linet’s mother describes her as a bright and intelligent girl with great potential. However, she reports that Linet has been frequently ill since childhood. On this particular occasion, Linet had gone to visit her elder sister when she suddenly fell sick. She was taken to a nearby dispensary, where she was found to be extremely frail and weak. Due to the severity of her condition, she was referred to Siaya County Referral Hospital for further evaluation and management. On presentation, Linet complained of generalized headache, mild dizziness, abdominal pain associated with vomiting, chest pain, prolonged low-grade fever alternating with chills, general body weakness, and intermittent nosebleeds. She also had an ulcer on her left lower limb. On physical examination, she appeared ill and weak. Abdominal examination revealed an enlarged liver and spleen, and she was noted to be jaundiced. Following clinical assessment and investigations, Linet was diagnosed with severe malaria, severe anaemia, hypokalaemia and hepatocellular injury, with the underlying cause of the liver injury yet to be established.
Medical history
Linet was brought to the hospital with complaints of headache, a wound on her leg and was extremely weak. She was diagnosed with severe malaria, severe anemia and hypokalemia. This prompted admission where she was transfused blood, administered AL, tramadol, paracetamol among other drugs. She responded well to medication and was eventually discharged on orals. A visit to the home revealed linet was recuperated fully and had gone back to her normal life.
Prognosis
Linet was diagnosed with severe malaria, severe anaemia, and hepatocellular injury, all of which are serious and potentially life-threatening conditions. She requires urgent medical intervention, including antimalarial therapy, blood transfusion, and supportive treatment for liver dysfunction. Following timely treatment and close medical monitoring, Linet’s condition she is showing gradual improvement. Although Linet’s initial condition was severe, her prognosis is cautiously favourable.
