Joseph is a 14-year-old boy growing up in severe socio-economic hardship. His family depends on irregular casual labour that provides very little and unpredictable income, making it difficult to meet basic needs such as adequate food, clean water, clothing, and safe shelter. They live in an overcrowded, poorly constructed house with limited sanitation and unsafe water sources, increasing the risk of illness. Due to financial constraints, the family often cannot afford transport to health facilities, consultation fees, laboratory tests, or medications, which leads to delayed or missed medical care whenever Joseph becomes unwell. Low levels of education in the household also limit awareness of disease prevention and early health-seeking, while food insecurity weakens Joseph’s immunity and increases his vulnerability to illness. Joseph presented with a swelling on the right side of his forehead near the eye, which was soft, fluctuant, immobile, and severely tender. The swelling has been present for several months but became painful and progressively worse over the past three days, accompanied by throbbing headaches and fever. The condition now interferes with his daily activities, schoolwork, and use of his right eye. He has been advised to undergo surgical excision of the suspected lipoma to relieve pain and restore normal function.

Joseph came to the hospital accompanied by her mother, with face swelling proximal to the right eye, it was a soft, fluctuant, severely tender and immobile in nature.This affects his normal day to day activity like studying and using the right eye. He was diagnosed with lipoma, was put under treatment and management and responded well to medication and discharged on stable condition.

Joseph was diagnosed with an infected lipoma on the forehead, presenting with increasing swelling, severe tenderness, fever, and throbbing headache. The infection had progressed to the point of significant pain and interference with vision and daily functioning, making it a medical urgency. He required prompt surgical excision and antibiotic treatment to control the infection and prevent further spread to nearby tissues, including the eye and surrounding structures. With timely surgical removal, antibiotics, and proper follow-up, Joseph’s prognosis is very good, and he is expected to recover fully and return to normal activities. However, without urgent treatment, the infection could worsen, leading to serious complications and prolonged illness and worse still extending to the eye
