Why is treatment sometimes done earlier than the payment?

When a case is urgent and vital, hospitals often proceed with delivering medical care right away to save a life, even before the finalisation of admission procedures. Usually, when this happens, hospitals put the bill on our credit until the funds are transferred to their accounts. That’s why the chronology can sometimes be in disorder.

About blood

Severe malaria is a life-threatening medical emergency caused primarily by Plasmodium falciparum parasites. It is defined by the presence of malaria parasites in the blood accompanied by clinical or laboratory evidence of vital organ dysfunction or metabolic abnormalities. Severe anemia is a critical medical condition where hemoglobin levels fall significantly below normal, severely limiting the blood's ability to transport oxygen to vital organs Hepatitis A is a contagious liver infection from the Hepatitis A Virus (HAV), spread fecal-orally through contaminated food, water, or close contact, causing symptoms like fatigue, fever, nausea, jaundice, dark urine, and pale stool

Siaya County Referral Hospital

Along siaya- Rangala road

Background

Elisha is the third-born child in his family. He lives with his parents and three siblings in a one-room mud-walled house. Both his parents are unemployed and rely on subsistence farming to support the family. When well, Elisha enjoys modelling with clay soil and reading books. According to his mother, Elisha initially complained of headache and fever. She took him to a community health promoter (CHP), where tests confirmed that he had malaria, and he was given antimalarial medication. However, the following evening, Elisha’s condition worsened dramatically. He began vomiting blood and developed nosebleeds, which alarmed his mother. She rushed him to a nearby health facility, where he was assessed and found to be critically ill. Due to the severity of his condition, he was immediately referred to Siaya County Referral Hospital for specialised care. Like many families from financially underprivileged backgrounds, Elisha’s mother initially hesitated about the referral because she feared that they would not be able to afford the cost of treatment at a higher-level facility. Recognising this concern, the nurse at the nearby health facility informed her about Helpster Charity, which supports vulnerable patients who are unable to pay for medical care. This reassurance enabled the family to proceed with the referral. At Siaya County Referral Hospital, Elisha underwent further evaluation and investigations. He was diagnosed with severe malaria, severe anaemia, and hepatitis A, all of which are serious and potentially life-threatening conditions requiring urgent medical intervention.

Medical history

Elisha was brought to the hospital with complaints of hotness of the body, vomiting blood and nose bleeds. He was diagnosed with malaria, severe anemia and hepatitis A. This warranted prompt admission, he was transfused blood, and administered IV Artesunate, IV ceftriaxone, pcm , flagyl among other drugs. He responded well to medication and was eventually discharged on orals. A visit to the home confirmed that he had well recovered and had gone back to school

Prognosis

The prognosis for a 6-year-old child diagnosed with this severe anemia, malaria and hepatitis A is generally favorable if the child reaches medical care quickly, with survival rates for treated severe malaria often exceeding 90–95%. The most critical period is the first 24 hours of hospitalization, where rapid intervention with IV antimalarials and blood transfusions is essential to address life-threatening anemia. Elisha's prognosis is favourable.
